Dance instruction may require an instructor to physically touch a dance student during class time for the purposes of correcting dance technique, and students or parent/guardian must agree to allow this touching, and hold harmless and release Dance Arts South, LLC from any liability regarding such.
Once a class has been dismissed a parent/guardian is responsible for promptly picking up the child and hold harmless and release Dane Arts South, LLC, from any liability after the child has been released.
I also acknowledge that I or my child is in sufficient physical and mental condition that will allow for participation in an aerobic exercise situation.
I agree to assume all risks and hazards incidental to the participation in dance activities, and to hold harmless Dance Arts South, LLC, against any and all injuries, illnesses, cost, losses, damages and expenses (including attorney's fees) which I might suffer from participation in any class, production, rehearsal or event of whatever kind or character and without limitation.
By signing this agreement I understand and I am waiving any claims, liens, demands or causes of action, which I may now or in the future possess against Dance Arts South, LLC, its employees, instructors, other students and advisors arising out of or in any way related to my participation in any production, rehearsal or event.
I the parent/guardian of the above listed child, hereby acknowledge that I will be entering into a Contract for Dance Classes/Fee Agreement with Dance Arts South. Dance Arts South, LLC agrees to provide dance instruction for my child in accordance with the mission, philosophy and policies outlined in the 2017-2018 Dance Arts South Information Guide.
Contract Start Date: 9/5/2017 Contract End Date: 5/5/2018
Contract Duration and Automatic Renewal: Contract is in force for 9 months from the Contract Start Date, with automatic renewal for subsequent months.
Agreement to Pay Recurring Fees: I the parent/guardian agrees to pay Dance Arts South monthly recurring fees as follows: 9 Month Tuition Auto-Draft in accordance with the Dance Arts South tuition fee schedule based upon total hours of the above listed child's class enrollment via parent/guardian authorized automatic checking account ACH charges. Alternatively, parent/guardian may prepay 9 months in advance E Check/ ACH, bank check, credit card or cash. Payment in monthly increments by any means other than E Check/ACH is not permitted unless alternative written arrangements are made in writing between Dance Arts South, LLC and the parent/guardian.
I understand that any requests for changes to the enrollment schedule or withdrawal listed herein must be made in writing and submitted to the Creative Director. Upon submission of enrollment, I am agreeing to compensate Dance Arts South for the above listed services as follows: Tuition is paid prior to the performance of dance instruction. Tuition is due whether or not the child attends the program. The tuition represents the child's place in the program. There is no credit given for vacation or emergency closure days, makeup classes will be scheduled to allow for missed dance instruction.
The Contract for Dance Classes/Fee Agreement may be cancelled by Dance Arts South at any time with or without notice, in its sole discretion. Any unused TUITION paid by the parents/guardians will be refunded within 30 days of cancellation.
The Contract for Dance Classes/Fee Agreement may be cancelled by the parents/guardians with two weeks written notice. Written notice of cancellation must be submitted to the Creative Director. There are NO REFUNDS for tuition paid, recital fees or dance costumes.
(a) I agree to purchase 9 Month Tuition Auto-Draft in accordance with the Dance Arts South tuition fee schedule based upon total hours of the above listed child's class enrollment via parent/guardian authorized automatic debit to my checking account each month for a contract period of 9 months.
(b) I hereby certify that I am an authorized signer on the bank checking account detailed below.
(c) I understand that I will be notified if my checking account payment fails to authorize for any reason, and that a $15 late fee will apply if I do not provide a valid credit card or checking account ACH information within 3 calendar days of the original rejection date.
(d) I understand that my child will not be able to attend classes if the account becomes 30 calendar days late.
I confirm that I have had the opportunity to discuss the information contained herein with a representative of Dance Arts South, that my questions have been answered fully and to my satisfaction and that I agree to abide by the conditions set forth herein as well as the policies contained herein by reference from the Dance Arts South Information Guide.
I agree to pay a $25.00 non-refundable registration unless a pre-registration form was submitted prior to 8/1/2018. Registration fees are processed via E Check/ACH bank draft upon enrollment.
I agree to pay a Recital Fee based upon the Dance Arts South fee schedule to be processed via E Check/ACH bank draft on 2/1/2018.

I agree to pay recital costumes fees based upon the costume pricing for each class my child is enrolled in for the 2017-2018 dance year. Costumes range from $60.00 - $150.00 per costume. Costume payments are processed via E Check/ACH bank draft on the following dates:
50% costume deposit - 10/14/2017
50% costume balance - 11/15/2017
CONTRACT FOR PRIVATE VOICE INSTRUCTION
I. GENERAL CONDITIONS
i. Lessons will be given over the academic year for which the student is registered. Students will be scheduled and parents notified of
recurring lesson date and time after registration.
ii. The student and parent(s) understand that this contract for Private Voice Instruction is binding for the current academic year and
will be renewed until written notice to discontinue services has been received.
II. ATTENDANCE AND TARDIES
i. The student and parent(s) understand and agree that lessons must run on schedule in order to not interrupt the flow of other lessons
scheduled with the same teacher or in the same room. The student will arrive on time to all scheduled lessons. Lessons will
end at their scheduled time, regardless of the time of the student's arrival.
ii. Students shall notify the instructor at least 24 hours in advance to cancel a lesson by calling or texting the instructor at 251-895-
6075 or the student will be charged for the missed lesson.
. a) Up to one (1) properly cancelled lesson a month may be rescheduled, at the instructor's discretion. Students are
financially responsible for any subsequent missed lessons regardless of notice.
. b) Missed lessons without proper notification, as a general rule, will not be rescheduled.
iii. If the lesson does not materialize within the first 10 minutes, all parties agree to treat the tardy as a missed lesson without proper
prior notice and will be not be rescheduled.
iv. The instructor shall begin lessons at their scheduled time, or the instructor shall extend the lesson to meet the required lesson time.
v. The instructor reserves the right to reschedule a lesson on a case-by-case basis, provided that significant reason is given in writing.
vi. Students shall participate, when possible, in scheduled events, studio classes, master classes, and recitals/Showcases. Recital
and/or Showcase dates will be announced at least two (2) months prior to the event, and may be held up to (2) times per
i. 30 minute lessons
a) 5 minutes: discuss progress and identify problems arising form previous week of practice.
b) 10 minutes: warm-ups and exercises
c) 10 minutes: repertoire (may be replaces with further exercises when necessary)
d) 5 minutes: Identify goals and objectives for upcoming week
ii. 60 minute lessons
a) 5 minutes: discuss progress and identify problems arising from previous week of practice.
b) 20 minutes: warm-ups and exercises
c) 30 minutes: repertoire (may be replaces with further exercises when necessary)
d) 5 minutes: identify goals and objectives for upcoming week
iii. Student agrees to bring the following to their lesson
a) Pencil with eraser
b) Notebook and any required sheet music
c) Bottled water, if needed
iv. Students agrees to complete the following between lessons
a) Practice, dividing time equally between warm-ups and working on repertoire
o 30 minutes, 3 times per week (beginning students)
o 45 minutes, 3 times per week (intermediate students)
o 60 minutes, 3 times per week (advanced students)
o These are minimum practice guidelines; the student should speak with their instructor
about adding additional days or times to this routine.
b) Acquire copies of any necessary music for upcoming lesson
IV. GENERAL RULES AND PROCEDURES
i. All students MUST remain in Dance Arts South lobby upon arrival until time for each lesson. It is the responsibility of the student
to arrive to the studio and keep the instructor informed of whereabouts when leaving the studio during scheduled lesson
ii. The Instructor reserves the right to terminate a private music lesson student if necessary.
iii. Parents agree not to interfere with lessons by refraining from attending lessons within the same room with the teachers and
students, calling out to students in rooms or interrupting teachers in rooms while lessons are taking place.
iv. Parents agree to remain courteous at all times and direct concerns or complaints to the Jamie Leatherbury, owner, Dance Arts
South, and not to teachers, other students, other parents or any other concerned personnel.
V. REGISTRATION AND PAYMENT
______i. A non-refundable registration payment totaling $25 is required at the time of registration to register the student and secure
the lesson. Early payments may be made if the parent wishes to do so.
_____ii. A payment totaling $25 is required at the beginning of each semester to offset the cost of music. Early payments may be
made if the parent wishes to do so.
______iii. All lessons are billed monthly. The parent or party responsible for the payment of the lessons agrees to a recurring charge
of $30 per 30-minute lesson or $60 per hour lesson. Parties agree that certain months will call for 5 lessons to be charged instead of 4.
______iv. Lessons are paid for by the month. All lessons payments are due on the 1st of the month and considered late on the 5th of
each month. Any payment not paid after the 10th of the month will be assed a late fee of $10.
VI. TERMINATION OR WITHDRAWAL
i. The student may request to be released from this contract for Private Voice Instruction but must provide a minimum of two (2)
week's notice. In the event that notice is not given and the student does not appear for lessons, Dance Arts South will hold
that student's place for a two-week period and the lessons will be charged as if they occurred but without the benefit of a
make-up requirement on the part of Dance Arts South. After two weeks of un-notified absences, Dance Arts South will
terminate this contract and no further lessons will be charged. NO REFUNDS WILL BE ALLOWED FOR UN-NOTIFIED
ii. Dance Arts South reserves the right to terminate a student's participation for reasons including, but not limited to, poor behavior,
theft or vandalism, use of inappropriate language, defiance or disrespect, fighting or any other behavior or action deemed
by the instructor to justify removal.
I have read and agree to be bound by the terms and conditions of this contract for private music instruction through Dance Arts South.
I fully understand that failure to abide by this contract will result in termination of this agreement and that privileges will be revoked
