Register for Sound Healing Class! This is for the session at 6 pm - 7 pm. The Sound Healing Session will be in-person at 330 W Cummings Park, Woburn, MA 01801. When you arrive you will see XC Art Center as the front, please walk in and the session will take place at the workshop room on the left. Acknowledgment of Risks & Informed Consent
I understand that sound healing sessions involve exposure to various sound frequencies and vibrations through instruments such as singing bowls, gongs, chimes, and tuning forks. I acknowledge and accept that:
Sound healing is a holistic wellness practice and is not a substitute for medical or psychological treatment.
Sound healing may induce deep relaxation, emotional release, or other physical and mental effects, including dizziness, discomfort, or heightened sensitivity to sound.

I have no known medical conditions or concerns that would prevent me from safely participating in a sound healing session. If I have a medical condition (such as epilepsy, pacemaker, hearing sensitivities, or mental health concerns), I have consulted with a healthcare professional before participating.

I understand that participation is voluntary, and I am responsible for monitoring my own well-being during the session.

Release of Liability
I hereby release, waive, and discharge Liyuan Ji and Nextgen Interarts Foundation from any and all claims, liabilities, demands, or causes of action related to my participation in the sound healing session. This includes, but is not limited to:
Personal injury, discomfort, or adverse reactions arising from the sound frequencies or vibrations.
Emotional, mental, or spiritual experiences resulting from the session.
Any pre-existing medical conditions that may be affected by the session.
Any loss, theft, or damage of personal property during the session.

Indemnification
I agree to indemnify and hold harmless Liyuan Ji and Nextgen Interarts Foundation from any legal claims or medical expenses arising from my participation in the sound healing session.

Medical Emergency Consent
In the event of a medical emergency, I authorize Nextgen Interarts Foundation to seek necessary medical assistance on my behalf. I understand that I am responsible for any medical costs incurred.
