Welcome to Liberty Gymnastics' Preschool Summer Camp!
Campers will enjoy working on basic gymnastics skills, playing games, doing arts and crafts, and making new friends! Camp T-Shirt is included in enrollment.

Campers will need to bring a NUT FREE snack each day.

Cost: $225 for 5-day Camp
Ages: 4-5 years old
Dates: Monday through Friday.
Hours: 9am - 12pm.

$100 non-refundable deposit due upon enrollment to hold camp spot. Balance will be charged 2 weeks prior to camp

Release and Waiver of Liability Agreement

In consideration of participating in Liberty Gymnastics Training Center's camp program I represent that I understand the nature of this Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity.

I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in the Activity.

I hereby release, discharge, and covenant not to sue Liberty Gymnastics Training Center, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.

I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extend allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

I understand that I have registered my child or children for recreational camps, and/or events at Liberty Gymnastics Training Center, Inc., also referred to herein as Liberty Gymnastics, located at 2330 Bates Ave (Suites A and B2/B3), Concord, California 94520. Liberty Gymnastics has a program whereby webcams are in use and my children are under streaming video surveillance that is used for training and management purposes, in addition to secure accessibility from the web in accordance with the terms and conditions associated with the Spot TV website, also referred to herein as Spot TV. I hereby consent to the photographing of myself and/or my minor child(ren) and the recording of my voice and/or that of my minor child(ren) when we are within the Liberty Gymnastics facilities. I agree that these photographs and/or audio and/or video recordings singularly or in conjunction with other photographs and/or recordings may be used for internal training, management of the facility, advertising, publicity, commercial or other business purposes. I further agree that the video footage is the sole property of Liberty Gymnastics and may only be used by Liberty Gymnastics. I agree that I have no rights to any of the video footage or photographs for any reason at any time. I also agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from the Content and Service. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal. Copyrighted work includes all web streaming and video/audio recordings. I understand legal action can be taken against me by Spot TV or Liberty Gymnastics for such copyright infringement. I understand that the term photograph as used herein encompasses both still photographs and motion picture footage. I assume full liability of all the terms outlined in this release for any family member's account activity associated with my child(ren).
