I give consent to my minor's participation in any or all of Utah Tumbling Academy's activities including, but not limited to: 1-on-1 Private Lessons, Semi-Private Lessons, Group Classes, Clinics, Events, Summer Camps, Birthday Parties, Indoor Playground, Open Gym and Field trips or any other activity sponsored by Utah Tumbling Academy. I acknowledge that I fully understand my minor's participation may involve risk of serious injury, including but not limited to, broken or fractured bones, sprained joints or ligaments, paralysis, and transmission of sicknesses. I understand that if I have any risk concerns, I have the right to place any concerns in writing and deliver said writing to UTA Management before I sign this agreement and before my minor's participation in the activity begins. Being fully aware of these dangers, I hereby give consent for myself and my child(ren) to participate in any Utah Tumbling Academy programs and activities for which they are registered and I ACCEPT ALL RISKS associated with this participation and fully release Utah Tumbling Academy from any liability or negligence associated with any activity taking place at Utah Tumbling Academy. Further, I hereby, recognize and acknowledge my child's participation in recreational activities may involve bodily injury to myself and/or my child. In consideration of my child being able to participate in such events, I, for myself, my child, my heirs, my executors and administrators, hereby voluntarily and knowingly indemnify and hold harmless, defend, release, waive, and discharge Utah Tumbling Academy and its owner, director and employees and volunteers from any and all suits, claims or liability, including negligence, based on any injury or claim that may result from my child's participation in Utah Tumbling Academy activities and classes. In addition, I agree that I or my insurance company will pay for medical, hospitalization or any other expenses resulting from my child's participation. All parents, guardians, and supervisors bringing students to Utah Tumbling Academy are strictly prohibited from accessing the tumbling floor mats. Utah Tumbling Academy is not liable for any injuries sustained by them on the mats. Parents, guardians, and supervisors acknowledge this when bringing students to our studio, waiving any claims against the Utah Tumbling Academy for injuries incurred on or near the mats. Exceptions apply to parents, guardians, and supervisors of children aged 2 to 3, who may accompany them onto the mats during designated classes. However, they assume all risks and waive claims against Utah Tumbling Academy for injuries. Parents, guardians, and supervisors must follow staff instructions on safety protocols and mat usage. Utah Tumbling Academy reserves the right to enforce this policy, with staff authorized to request compliance and take appropriate action. By bringing children to Utah Tumbling Academy, parents, guardians, and supervisors agree to abide by this policy, ensuring safety standards and a positive experience for all participants. I affirm that I am of legal age and am freely signing this agreement.

I hereby authorize Utah Tumbling Academy staff to act on my behalf in accordance with their best judgment in case of an emergency involving my child, and agree to assume full responsibility for all expenses, medical or otherwise, that may arise there from. I understand that I or my insurance company will pay for such emergency treatment.

Dress code for girls is a leotard with or without shorts. If your child does not own a leotard any type of tightly fitted activewear will work. Hair must be pulled back. Boys can wear gym shorts and a t-shirt. NO SOCKS, JEANS, TIGHTS, SKIRTS OR LOOSE CLOTHING.

If your child or anyone in your family has been sick within the past week, we ask that you do not attend. We appreciate your understanding.

We would like to gently remind families of the importance of extreme caution when using the parking lot. Not only is it imperative to drive through the parking lot slowly since children and parents are walking to and from their cars, but it is also important to pay even more attention while backing out. We want to make sure that we keep all children safe and we need all drivers to take extra precautions just as you would in a school parking lot.

It is the responsibility of Utah Tumbling Academy to offer professional instruction to your child while in class. We are not responsible for the care of students before or after class. Students are not to be left at the studio for excessive periods of time before or after class.
