Dance, along with the rigorous training and performance demands it entails, places significant physical stress on the body and carries the inherent risk of injury. On behalf of my child and myself (or, if I am no longer a minor, on my own behalf), I acknowledge and assume these risks. I agree that Cary Ballet Conservatory (CBC) and its staff shall not be held liable for any injuries sustained during participation in any CBC activity.

I understand that dance instruction may involve kinetic corrections, which can include physical contact as part of regular classwork and rehearsals. Students with concerns about this should communicate with their instructors before the class begins.

Furthermore, I acknowledge the risks associated with participating in CBC programs and assume full responsibility for any injuries, damages, loss, or death that may occur as a result of involvement in any related activities. I waive and release all claims against Cary Ballet Conservatory, its owners, faculty, staff, and consultants arising from participation in any program.

In the event of an accident or illness, I consent to emergency medical care for myself/my child as deemed necessary by ambulance or hospital personnel.

Students under the age of 12 must not be left unattended at the studio more than five minutes before the start of class. CBC does not provide staff to supervise students in the lobby, café, or restroom areas.

Students will not be permitted to wait outside the building after class until their ride has arrived. If your student is under the age of 10, please come inside the building to pick them up.

All students must wear appropriate cover-ups when arriving at or leaving the building. Wearing only a leotard and tights outside the studio is not permitted. Please ensure students wear shorts, pants, skirts, t-shirts, sweatshirts, or dresses over their dance attire.

CBC is not responsible for dancer supervision outside of its premises, including the plaza parking lot and nearby shops.

By signing this agreement, I acknowledge and agree to abide by the policies outlined above.

Withdrawal requests submitted after May 1 are not guaranteed a full or partial refund or account credit. Partial account credits for medical or hardship cases may be considered on a case-by-case basis and may require supporting documentation. If a late withdrawal results in lost resources for Cary Ballet Conservatory, a cancellation fee may be applied.

Account credits expire after one year and may be applied toward future CBC expenses for the same dancer or an immediate family member.

Failure to attend or participate in a program does not entitle the student to a refund or credit.

All withdrawal requests must be submitted in writing via the withdrawal form.

By enrolling, I acknowledge and agree to these policies.

Cary Ballet Conservatory reserves the right to photograph, film, and record students during classes, rehearsals, performances, and approved events. Any images or recordings taken by Conservatory staff or an approved photographer may be used, published, or reproduced at the Conservatory’s discretion.

All publicity during the term of this agreement shall be managed by Cary Ballet Conservatory, and the student agrees to participate in promotional activities as directed. The parent(s)/guardian(s) or student (if of legal age) grants permission for the student’s name, photograph, voice, image, and relevant information to be used in any publications, promotional materials, marketing campaigns, advertising, or on the Conservatory’s website without compensation or prior notice.

Photographs and videos may also be taken during open classroom sessions or approved events. Additionally, the parent(s)/guardian(s) or student allows media interviews to be conducted on campus or at school-related events.

By signing this agreement, the parent(s)/guardian(s) or student releases and holds Cary Ballet Conservatory harmless from any liability arising from the use of the student’s name, photograph, voice, image, or information.

Furthermore, all online content, including but not limited to Zoom classes, social media broadcasts (Facebook, Instagram, Twitter, YouTube, podcasts, blogs, websites), and choreography shared or created by the Conservatory, remains the exclusive property of Cary Ballet Conservatory. Any work created by an employee within the scope of their employment is also considered Conservatory property.
