I understand that there are specific risks of physical or property damages, losses, illnesses, or injury that may result from my or my child's participation with Studio K Dance Collective, LLC and I voluntarily assume the risks associated with such participation.

Studio K Dance Collective, LLC is not responsible for articles lost, stolen, or left on the premises. Studio K Dance Collective, LLC is not responsible for any accident, injury, or illness. Studio K Dance Collective, LLC is not responsible for children left unattended on the property.

Dance is an activity involving stress and risk, therefore students must be in good health to participate. Please inform instructor of any personal limitations you may have. If you have any doubt to your physical abilities, please consult your physician before enrolling. Students may decline to participate in any activity.

I give my child(ren) permission to participate in the class(es) indicated on our enrollment application. I realize every precaution is taken to eliminate any injuries or hazards and that a competent instructor is present; however, in the event of an injury I hereby waive, release and hold harmless Studio K Dance Collective, LLC from any liability, for damages or claims for damages for personal injury, as well as for property damage which arise in conjunction with our classes, rehearsals, performances, or travel to and from off site rehearsals and performances.

I agree to waive and release Studio K Dance Collective, LLC, its officers, agents, employees, and volunteers from and against any claims, costs, liabilities, expenses, or judgments including attorney's fees and court costs arising out of participation in Studio K Dance Collective's programs or any illness or injury resulting there from and hereby agree to indemnify and hold harmless Studio K Dance Collective, LLC from and against any and all claims.

Studio K Dance Collective, LLC shall have the right to use the names, photographs, videos, or other likeness of my child(ren) and to exhibit the same through any medium for advertising, promotional, or commercial purpose. All such reproductions shall be the exclusive property of Studio K Dance Collective, LLC
