Dance Intensive is for students 11 years old and up, Monday-Friday 12:30-3:00PM. Students should wear leotard, tights, and have ballet, tap and jazz shoes. Please bring water bottle and small peanut free snack if desired.
Fee $100.00 ($5 discount if paid by May 15-June camp and June 17 - July camp).
Check in will be in the B220 hallway and you will be directed to the correct dance room.
Credit card on file or entered at time of registration will be charged after registration is processed.
Full refund if notified by email prior to May 1 for June Camp and June 1 for July Camp.
1/2 refund if notified by email by May 15 for June Camp and June 15 for July Camp.
No refund for any later date.

In consideration for being allowed by First Baptist Church Woodstock (FBCW) to participate and/or attend any church sponsored Dance for His Glory activity.
I agree to release, discharge, and hold harmless Dance for His Glory and FBCW, its employees, agents, and members from any and all claims or demands due to personal injury, illness, or death as well as any and all property damage sustained of any nature which might be incurred by me while participating in dance classes. I also agree to be directed and responsible to the dedicated church leadership for the event or activity. Further, I agree to hold harmless and to indemnify Dance for His Glory and FBCW, its employees, agents, or members for any liability or expenses sustained by the church as a result of my participation. If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable arbitration process.
I hereby authorize Dance for HIs Glory and FBCW and its representatives to initiate any medically necessary care on my behalf in the event of my incapability to present myself for such care and agree to be financially responsible for any incurred expenses.

Photo Release

Dance for His Glory requests your permission to photograph or video your child during the session. Photos and/or videos will be used for possible promotional materials for the Dance for His Glory program, website and/or social media. Covid - 19

As any other public place, we cannot guarantee that your child will not be exposed to Covid 19. By agreeing you agree to waive any claims of liability against D4HG and to hold D4HG harmless if your child contracts Covid - 19 despite our best efforts to prevent it. Additionally, I agree to notify D4HG if my child or any member of my household contracts Covid - 19 or is quarantined due to coming in contact with someone who has tested positive. I agree that my child and any accompanying persons will not enter FBCW if we exhibit any of the following new or worsening signs or symptoms of possible COVID-19: A temperature greater than 100.4 degrees Fahrenheit, cough, shortness of breath or difficulty breathing, other respiratory symptoms, or at least two of the following symptoms: chills, repeated shaking with chills, muscle pain, sore throat, runny nose, nausea, diarrhea, vomiting or new loss of taste or smell. I also agree that, neither my child(ren) nor any accompanying adult will enter FBCW if we have had known close contact with a person who is lab-confirmed to have COVID-19.
