Welcome to our Living Arts Dance Family! This is our online registration form for Living Arts Summer Dance 2024. To reserve your child's space in this Summer Program a $55.00 registration fee must be paid at the time of completing this form. All registration payments are nonrefundable. A registration fee of $55.00 is required to reserve my child's space in camp, this fee is nonrefundable and nontransferable. 1 uniform will be given to my child on their 1st day of camp.*
Tuition is due in full per session. 2 week camp minimum consecutive weeks attendance required. *

I understand that once I select the weeks below, those dates will be added to my dancers account and the space is not held until tuition is paid. Camp spaces are first come first served.*
My child is Attending Comp Team Session: 6/10/24 - 6/14/24 & 6/17/24 - 6/21/24 4:45 pm - 8 pm. Disney Intensive Session Rate: $250 due 6/3/24*

End of the Summer Performance: I understand that Dancers must attend Session 3 to participate in the summer performance. My child is participating in the End of the Summer Performance.*
My Child has permission to attend swim days and studio activity days on Friday's. Parents will be provided with the full trip itinerary by Orientation.*
I understand this is registration for a dance camp, my child will need dance supplies: Living Arts Dance Uniforms, Tan Dance Tights, Tan Colored Canvas Ballet Shoes & Black Tap shoes for camp daily. *
