PARTICIPATION AGREEMENT

As the legal parent or guardian, I understand that in participating in any dance class, workshop, rehearsal, or performance, there is the possibility of physical injury. Dance Unlimited, LLC will make all reasonable efforts to help our students avoid injury. As the legal parent or guardian, I hereby indemnify, release and hold harmless Dance Unlimited, LLC , its owners, and its operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Dance Unlimited, LLC , its owners and operators or in route to or from any of said premises. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Dance Unlimited, LLC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Dance Unlimited, LLC program.
Further, I hereby indemnify, release and hold harmless Dance Unlimited, LLC , its owners, and its operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while receiving instruction virtually by Dance Unlimited, LLC. I hereby acknowledge and agree to be bound by all of the terms set forth in this Liability Waiver and Acknowledgement of Risk I further agree to assume all of the risks and responsibilities for any injury, or accident which might occur to my child during any class, workshop, rehearsal, performance, or activity. Furthermore, I understand that I should be aware of my child's physical limitations and I agree not to permit my child to exceed such limitations.
