I agree that photographs and/or video as well as contact information of/for myself (or my dancer) can be shared with the schools participating in the National Summer Intensive Audition tour.
I hereby grant the participating schools permission to use photographs and video of myself and/or my dancer for evaluation and administrative purposes without payment or any other consideration. I hereby irrevocably authorize the participating schools to edit and copy photos and video for purposes of evaluating and administering the auditions. In addition, I waive the right to inspect or approve the photos and video, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph or video.
In the event that a school is unable to hold its in-studio audition due to changing COVID-19 restrictions or weather, registered students may still participate virtually and send in an audition video. We understand that taking class at home is not the same as being in a studio, and that not everyone has easy access to a good place to take class. The audition class will be modified to accommodate this challenge, and students should send in a video of their classwork [see audition video guidelines]. Participating NSIA schools will observe both the virtual class and the videos of those students. This option is exclusive to students who are registered for an in-person NSIA audition which has been cancelled due to unforeseen circumstances, and will not be offered to additional students or those pre-registered students who decide not to attend the in-person audition. Please note that each audition has a limited capacity (6-20 students), which is based on the current COVID-19 restrictions of each city. There are no refunds for NSIA audition fees.
The secondary registration form sent within the confirmation email must be completed in order to be considered by observing schools.

I am aware that ballet dancing and the gymnastic exercise associated with it place unusual stress on the body and carry with them the risk of physical injury. On behalf of my child and myself, I assume the risk and agree that the Kentucky Dance Council Inc., d/b/a Louisville Ballet , and its Board of Directors, faculty, chaperones and agents shall not be liable in any way for any injuries sustained or loss of property during attendance at the Kentucky Dance Council Inc., Louisville Ballet activity or any of its related functions. Permission is granted for emergency medical treatment for my child / me.

Audition Day Agreement

Please arrive 15 minutes prior to the start of the audition to check-in and receive an audition number. All dancers should bring a current head shot and recent photo of first arabesque (en pointe for ladies).
Ladies should wear a solid color leotard, pink tights, ballet and pointe shoes.
Gentlemen should wear a white shirt, black or grey tights and ballet shoes.
The audition class lasts 90 minutes and consists of barre and center work. Ladies with 2 years of pointe experience should be prepared to go en pointe immediately after barre for the center portion and remainder of the class.
Audition results are emailed within 12 days of your audition. Each school will reach out to students with the results of their audition no later than 2 weeks following the final audition. Students may receive results before that deadline.
