Public Release

I hereby grant to School of Sacramento Ballet the absolute and irrevocable right and unrestricted permission to use my/my child's name, likeness, image, and/or appearance as such may be embodied in any photos, video recordings, digital images, and the like, taken or made on behalf of School of Sacramento Ballet or its partners. I agree that School of Sacramento Ballet has complete ownership of such material and can use said material for any purpose consistent with School of Sacramento Ballet's mission. These uses include, but are not limited to, videos, publications, advertisements, news releases, Web sites (including social media), and any promotional or educational materials in any medium.
I acknowledge that I and my child will not receive any compensation for the use of such images, video, likeness, etc. I hereby release and discharge School of Sacramento Ballet, and its agents, representatives and assignees from any and all claims and demands arising out of or in connection with the use of my or my child's name, likeness, image, and/or appearance, including any and all claims for invasion of privacy, right of publicity, misappropriation or misuse of image, and/or defamation. This release shall be binding upon me, my heirs, legal representatives, and assigns.
This agreement is being made and entered into under the laws of the State of California and shall be governed and interpreted in accordance with the laws of said state. This agreement embodies the entire agreement of the parties (subject and photographer). No modification of this agreement shall be of any effect unless it is made in writing and signed by all of the parties to the agreement.

Medical Release

I hereby give my permission to the Administration, faculty, and staff of Sacramento Ballet to authorize any emergency medical care that may be required during my or my child's participation in classes, performances, or any related Sacramento Ballet events. This authorization extends throughout the entire year, or until that student is no longer enrolled with The Sacramento Ballet, whichever comes first. I understand that I am responsible for any and all charges as a result of such care or medical treatment.

Covid-19 Release

Parent and student acknowledges that despite additional federal, state, and other preventative guidelines implemented by Sacramento Ballet as a direct response to the novel coronavirus ("COVID-19"), that COVID-19 is extremely contagious and can be spread from person-to-person contact. Participant further acknowledges that those engaged in ballet classes are not immune from COVID-19 or the associated risks thereof. Parent and student is aware of the risks, dangers and hazards associated with COVID-19, and knowingly and freely accept and fully assume all such risks, dangers and hazards and the possibility of personal injury.

Code of Conduct

Parents and students agree to:
Respect all members of School of Sacramento Ballet community.
Conduct themselves, whether inside or outside of organization, in a manner that is reflective of the discipline and etiquette required in ballet.
Acknowledge that the Organization's discipline is based on a fundamental respect for persons, property, authority, and the mission with objectives of the organization.
Whereas the School recommends a no use policy regarding social media, any contact of classmates in a group chat or other groups on social media which does not include all and/or speaks poorly of the organization or others is strictly forbidden.
Bullying, harassment, or discrimination, including via social media, will not be tolerated. Students or adults who witness bullying, harassment, or discrimination should report it to the School immediately.
The School reserves the right to determine, at its discretion, which actions fail to meet the principles and philosophy of the School. Failure to follow these guidelines may result in a warning, disciplinary action, suspension or dismissal depending on the severity of the offense.
