Severe acute respiratory syndrome coronavirus 2 ("SARS-CoV-2"), which is responsible for the novel Coronavirus Disease ("Covid-19") has been declared a worldwide pandemic by the World Health Organization. COVID-19 is highly contagious and is believed to spread mainly from person-to-person contact. TANYA'S DANCE CO., has put into place preventative measures intended to reduce the spread of COVID-19: however, TANYA'S DANCE CO. cannot guarantee that you, your child(ren), and family will not become exposed to , or infected with, COVID-19 and/or any mutation or variation thereof while attending or participating in classes, recitals, private lessons, parties, or any other activities at TANYA'S DANCE CO. studios. By checking the box below and signing below, you as the participant, or legal parent or guardian, as the case may be, voluntarily agree to accept all of the risks of exposure, infection, injury, illness, death, damage, loss, liability, or expense of any kind relating to COVID-19 by participating in, and/or having your child participate in the aforementioned activities. You, on your own behalf and/or on behalf of your child, hereby release, pledge not to sue, and to indemnify, and hold harmless TANYA'S DANCE CO., its owners and operators, from any and all liability, claims, demands, damages, cost, expenses of any kind, and causes of action whatsoever arising out of or related to exposure or contraction of COVID-19 and/or any mutation or variation thereof. By signing this Agreement and checking the box below, you acknowledge that you understand and agree that this release includes any claims based on the actions (negligent or not) of TANYA'S DANCE CO., its owners, operators, contractors, employees, instructors, agents, and/or volunteers regardless of whether a COVID-19 infection occurs before, during, or after participation in any Organization activity. By signing this Agreement and checking the box below, you further agree and acknowledge that you will not send your child to TANYA'S DANCE CO. in the event that they, or any household member, tests positive for COVID-19, or is showing symptoms consistent with COVID-19. TANYA'S DANCE CO. has the right to remove your child from in-person classes if they are unable to abide by the procedure of this document.
