GENERAL TUITION POLICIES:
I understand that I am responsible for the tuition for classes for which I have registered for.
Tuition and all fees are non-refundable/non-transferable. The School of Philadelphia Ballet is not liable or obligated in any way to process any refunds or issue any tuition credits.
INITIAL PAYMENT:
I hereby authorize Philadelphia Ballet to initiate a charge from the designated charge/debit card input upon enrollment.
PREAUTHORIZATION:
I hereby authorize Philadelphia Ballet to initiate preauthorized electronic funds transfers and debit the authorized amount indicated at the time of registration from the designated charge/debit card input upon enrollment.
All outstanding balances will be charged to the credit card on file.
All charges returned due to insufficient funds or expired cards will incur a $25 fee.
I agree to notify Philadelphia Ballet in writing of any changes in my account information.
I will not dispute Philadelphia Ballet recurring billing with my credit/debit card issuer/bank so long as the debit amount and frequency corresponds to the terms indicated in this contract.

SPB Covid-19 Vaccine Policy Requirement

To ensure the health and safety of our students, their families, employees, and members of our community, the School of Philadelphia Ballet will require that all students be fully vaccinated with the primary series and up to date on vaccine booster dose(s) when eligible.
Overview
This policy is designed for use together with, and not as a substitute for, other safety measures to prevent the transmission of COVID-19 in the school.
As a condition of enrolling, all students must: (1) be fully vaccinated against COVID-19 (i.e., two weeks following receiving both doses of a two-dose vaccine or one week after receiving a single dose of a one-dose vaccine considered effective by the Centers for Disease Control and Prevention (“CDC”); and (2) provide the Philadelphia Ballet Human Resources Department with vaccination details, unless the student has obtained an approved exemption as an accommodation. The process for seeking an accommodation is explained below.
Philadelphia Ballet also requires that students receive any future booster shots that may be recommended by the health authorities.
To confirm or provide your vaccination status, please send a copy of your vaccination card directly and confidentially to the School Registrar at amedford@philadelphiaballet.org. Records will be kept confidential. Once you submit your proof of vaccination to the School Registrar, we recommend you immediately delete your vaccination record from your local computer drive after uploading. If this step is not taken, Philadelphia Ballet cannot guarantee the privacy and security of that information.
Employees of Philadelphia Ballet are required to be vaccinated as well. Independent Contractors, guests and visitors will be asked for proof of vaccination when working for Philadelphia Ballet and/or visiting the workplace, attending rehearsals, performances and/or events. No records will be maintained by Philadelphia Ballet for individuals that fall within these categories.
Documentation should only include proof of vaccination and no other medical or genetic information (e.g., family medical history).
Exemptions
Any current student who cannot receive a COVID-19 vaccination because of a qualifying disability or sincerely held religious belief should contact Human Resources to request an accommodation. Newly enrolled students who cannot receive a COVID-19 vaccination because of a qualifying disability or sincerely held religious belief should contact Human Resources to request an accommodation within seven days from the initial date of registration.
Once a request for reasonable accommodation is made, Philadelphia Ballet will engage in a case-by-case interactive process to assess whether such student is eligible for an exemption from the vaccination requirement under the same analysis as afforded to EEOC- and ADA-mandated exemptions from workplace requirements in accordance with federal, state, and local law. Philadelphia Ballet will keep any medical information obtained in connection with a request for a reasonable accommodation confidential in accordance with, and to the extent required by, applicable law, regulations, and guidance.
This policy is an essential component of Philadelphia Ballet’s overall strategy and commitment to maintaining a safe and healthy school and workplace during the COVID-19 pandemic. This policy will remain in effect until further notice.

PB Guidelines for Healthy & Safe Operation of SPB: Part 1

This policy document is intended to reflect the mission of School of Philadelphia Ballet, provide information and guidance to help understand best practices in the current environment to prevent transmission of COVID-19 infection, and provide students with steps to protect themselves and others in the studio space. The primary sources for this document are from the City of Philadelphia, Philadelphia Department of Public Health (PDPH), the Centers for Disease Control and Prevention (CDC) Pandemic Response, and the guidelines offered by our partner unions. As these sources provide up-to-date, reliable information on COVID-19, those revisions will be reflected in updates to students.
Our COVID-19 Task Force is committed to building relationships with the local Philadelphia Department of Health Coronavirus taskforce, staying apprised of information from experts in the field and other cultural organizations, reviewing internal policies & feedback regularly, and overseeing safety training and implementation at our facility.
Our primary goal is to safeguard the health of our students, reduce transmission among students, protect people who may be at higher risk for adverse health complications, maintain business operations, and minimize adverse effects on others with whom we interact in the course of doing business.
All our stakeholders (staff, dancers, faculty, students, parents, and Board of Trustees) are expected to inform themselves of our policy and respect the safety measures in place. We will continue to plan and implement ongoing safety measures in accordance with guidance and direction of government health authorities. We welcome your feedback and ideas to ensure increased health and safety, as well as individual compliance.
RISK MITIGATION
Administrative Controls to Prevent Transmission
All faculty, dancers, staff, and independent contractors engaged by School of Philadelphia Ballet must be fully vaccinated with the primary series and up to date on vaccine booster dose(s).
All students eligible to receive the vaccination must be fully vaccinated and up to date on booster dose(s) to participate in classes and performances.
All visitors and guests to our North Broad studios must provide proof of vaccination and booster to enter. ? If anyone does not have a vaccination card and photo ID, they will not be permitted into the building.
Independent Contractors, guests and visitors will be asked for proof of vaccination when working for School of Philadelphia Ballet and/or visiting the workplace, attending rehearsals, performances and/or events.? No records will be maintained by School of Philadelphia Ballet for individuals that fall within these categories.
For Purposes of this policy:
Fully Vaccinated– people are considered fully vaccinated for COVID-19 two weeks or more afterthey have received the second dose in a 2-dose series (Pfizer-BioNTech or Moderna), or more than 2 weeks afterthey have received a single-dose vaccine (Johnson and Johnson (J&J) /Janssen).
Up to date?- means a person has received all recommended COVID-19 vaccines, including any booster dose(s)?when they are eligible.
Medical and religious exemptions may be requested through Human Resources and will be considered on a case-by-case basis.
Students, visitors and/or guests coming to the building agree to self-monitor for signs and symptoms of COVID-19 (symptoms typically include fever, cough, and shortness of breath):
if he/she is experiencing symptoms of COVID-19 he/she may not enter the building.
if he/she experiences symptoms of COVID-19 within 5 days of visiting the building, the individual must notify the School of Philadelphia Ballet at school@philadelphiaballet.org.

PROTOCOL FOR WHEN A STUDENT HAS A COVID-LIKE ILLNESS
Any student with symptoms of COVID-like illness should not come to class. If symptoms occur while in class, the student should be isolated and then sent home immediately. They should be advised to seek medical care if symptoms are severe.
Symptoms of COVID-19 include cough, fever, shortness of breath, fatigue, chills, headache, sore throat, muscle pain, new loss of taste or smell, and diarrhea.
Students must notify the Registrar (school@philadelphiaballet.org) if they will be out of the studio due to illness.
If students have minor symptoms or are unsure whether they have acute respiratory illness, they will stay home, get a COVID-19 test, and check with a medical professional before coming to class.
Symptomatic students may return to class after they meet ALL the following criteria:
at least 1 day fever-free (off fever reducing medications like Tylenol or ibuprofen) and
at least 5 days have passed since the symptoms started and
symptoms have significantly improved and
a well-fitting mask is to be worn for 5 additional days at all times when around other people.
If you have access to antigen tests, you should consider using them. With two sequential negative tests 48 hours apart, you may remove your mask sooner than day 10.
QUARANTINE/ISOLATION PROTOCOLS
Students who have tested positive for COVID-19 (using a PCR or rapid antigen (Home Kit) test) must isolate for 10 days from the start of symptoms or, if asymptomatic, from the date of the positive test.
Students with a confirmed case of COVID-19 who are fully vaccinated and up to date on booster dose(s) and have completed at least 5 days of isolation and are symptom free may return to class on day 6 and must continue to wear a well-fitting mask for 5 additional days at all times when around other people.
If you have access to antigen tests, you should consider using them. With two sequential negative tests 48 hours apart, you may remove your mask sooner than day 10.
Should symptoms return - do not come to class; get evaluated by a medical professional; contact your supervisor/manager and HR to determine appropriate protocols.
Students who have had close contact with a confirmed COVID-19 case in a non-household setting must self-monitor for symptoms, isolate if symptoms develop, and continue to wear a well-fitting mask when around others. Close contact is defined as:
Being within approximately 6 feet (2 meters) of a COVID-19 case for a prolonged period of time (15 minutes or longer in a 24-hour period)
Having direct contact with infectious secretions of a COVID-19 case (e.g., being coughed on)
Students fully vaccinated and up to date on booster dose(s) who are household contacts of a confirmed COVID-19 case and do not become ill must wear a well-fitting mask for 10 days when around others, test at 5 days using a rapid antigen starting with when the ill household member tested positive or when their symptoms began. They do not have to quarantine as long as they remain symptom free and negative for COVID-19.
Students who are fully vaccinated and up to date on booster dose(s) who were in the same indoor environment with a COVID-19 case, but not meeting criteria for a close contact (i.e., being in a room more than 6ft away from the case) should self-monitor for symptoms for 10 days after their last contact with the case and isolate if symptoms develop.
Students who have had contact with someone who has been exposed to a case or possible case are not required to isolate unless that person develops symptoms or is diagnosed with COVID-19.
Follow the cleaning protocols above if the student with COVID-19 has been present in the studio while sick or within 48 hours prior to developing symptoms.
*All persons who exhibit new or unexplained symptoms of COVID-19, regardless of vaccine status, still need to isolate and take a PCR or Antigen (Home Kit) COVID

COVID Liability Waiver

ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. The Philadelphia Ballet (PAB) has created safety guidelines for in-person training. The attached guidelines were developed through research in consultation with the Philadelphia Department of Public Health, the Center for Disease Control and Prevention (CDC), state and local officials, as well as our own research in our particular field for operation, including in consultation with our colleagues from dance schools across the nation (the "PAB Guidelines for Healthy and Safe Operation of the School of Philadelphia Ballet"). Philadelphia Ballet has put into place these preventive measures to reduce the likelihood of spread of COVID-19. However, PAB cannot guarantee that you or your child (the "Student") will not be exposed to COVID-19 while on PAB's premises. By reading the PAB Guidelines for Healthy and Safe Operation of the School of Philadelphia Ballet and adhering to the practices outlined therein you understand the risk and contagious nature of COVID-19. By signing this release, you acknowledge the risk of COVID-19 transmission to you or your child by attending the School of Philadelphia Ballet (the "SPB") or otherwise visiting PAB's premises. This exposure may result from the actions, omissions, or negligence of PAB and others, including but not limited to, faculty, staff, and other students and their families. You further agree that you and your child will comply with all protective measures and protocols implemented by PAB and/or established by the CDC and the Commonwealth of Pennsylvania. READ CAREFULLY AS EXECUTION OF THIS FORM WILL WAIVE LEGAL RIGHTS: By checking the "I've read the above and agree" box below, you acknowledge that: I understand that PAB has implemented the enhanced safety and sanitation protocols (outlined in the PAB Guidelines for Healthy and Safe Operation of the School of Philadelphia Ballet) in response to COVID-19. Student and his/her assignees, heirs, parents, guardians, and legal representatives hereby voluntarily release, forever discharge, and hold harmless PAB and each of their officers, trustees, employees, faculty, staff, students, agents, representatives, or groups affiliated therewith (collectively, the "Released Parties") from any liability, claims, or causes of action of whatever nature, arising from or related to exposure to COVID-19 for any reason whatsoever to the fullest extent permitted by law, including without limitation any liability, claims, or causes of action based on the actions, omissions, or negligence of the Released Parties. Photo Consent

I hereby consent to and authorize the use and reproduction by Philadelphia Ballet and the School of Philadelphia Ballet of any and all photographs, recordings, videotapes and/or other reproductions of my child's likeness for any purpose, whatsoever, without compensation. All images shall constitute the property of Philadelphia Ballet and the School of Philadelphia Ballet, solely and completely. Medical Consent and Liability Waiver

I am aware that dance and the nature of the training and performing associated with the School of Philadelphia Ballet place unusual stress on the body and carry with them the risk of physical injury. Authorization for Substituted Consent and Emergency
