BY SIGNING THIS REGISTRATION FORM, I AGREE TO READ AND COMPLY WITH THE POLICIES AND PROCEDURES OF ELEVATE DANCE CENTER.

I do voluntarily and knowingly execute this release with the expressed intention of effecting the extinguishment of and complete release from any and all claims, actions, demands or rights to monetary judgement arising from any and all injury or physical harm which may arise from or be sustained as a result of my participation of my child and/or legal ward in various programs of instruction, practice and physical activity associated with the study of dance and related activities conducted at the Elevate Dance Center.

This will be a CLOSED audition.

GROUP 1: Age 8 years old and Under: 10:00 am - 11:00 am
GROUP 2: Ages 9 - 12 years old: 11:00 pm - 12:00 pm
GROUP 3: Ages 13 years old and Over: 12:00 pm - 1:00 pm

**Times are approximations and can vary dependent upon the number of auditionees, etc.

There will be a designated warmup area available for auditionees 30 minutes prior to audition start time.

Girls: Black leotard and pink tights with a proper ballet bun. Black booty shorts or black leggings may be worn for jazz.

Boys: Fitted black jazz pants, ballet tights or shorts and a fitted white shirt. Hair geled and styled.

Shoes: Ballet, Jazz

I am aware the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact.
Elevate Dance Center (hereafter referred to as "EDC") has put in place preventative measures to reduce
the spread of COVID-19; however, Elevate Dance Center cannot guarantee that I will not become infected with
COVID-19. Further, participation in any activity could increase my risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily
assume the risk that my family may be exposed to or infected by COVID-19 by participation; and that
such exposure or infection may result in personal injury, illness, permanent disability, and
death. I understand that the risk of becoming exposed to or infected by COVID-19 at Elevate Dance Center may result
from the actions, omissions, or negligence of myself and others, including, but not limited to, EDC
employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any
injury to myself (including, but not limited to, personal injury, disability, and death), illness,
damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in
connection with my participation at Elevate Dance Center. On my behalf, I hereby release, covenant not to sue,
discharge, and hold harmless EDC, its employees, agents, and representatives, of and from the Claims,
including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating
thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or
negligence of EDC, its employees, agents, and representatives, whether a COVID-19 infection occurs
before, during, or after participation.
