I agree that I am the parent or legal guardian of the above named participant, and am over the age of 18. I certify that my child has my permission to attend and participate in all activities and release Step Out! from blame for any injury that may occur as a result of their participation.

Release of Liability

I realize that participation in Arts classes and physical activities could result in some possible personal injury. Despite precautions being taken by Step Out!, accidents and injuries may occur. By signing this release form, I/we (the student and parent/guardian) assume all risks related to the use of any and all spaces used by Step Out!. I/we agree to release from responsibility Step Out! of Squamish, BC, including all teachers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Step Out!, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes.

Medical Emergencies

I authorize the staff or person in charge to summon an ambulance for emergency medical aid, should in the opinion of the person in attendance, feel such services are required and I cannot be contacted by phone. If such an emergency should arise, I shall be notified as soon as possible. I agree that any cost incurred for such services shall be the sole responsibility of myself.

Health and Safety Waiver

At Step Out! safety is our top priority and we are following social distancing and cleaning protocol. As any other public place, we cannot guarantee that your child will not be exposed (whether at Step Out! or elsewhere) or if exposed, that your child will not contract a disease. I agree to waive any claims of liability against Step Out! and its staff, and to hold Step Out! harmless if my child contracts COVID-19 or any other disease, despite Step Out!'s best efforts to prevent it. Additionally I agree to notify Step Out! if my child or any member of your household contracts COVID-19 or has come into contact with someone who tested positive for COVID-19. This action will help ensure the utmost safety of our community.

Payment Policies

I agree that the camp will be paid for in full before the start. Late or missed payment will result in a late payment fee of 10% of the total, per week overdue. If payment in full is not viable, payment plans can be put in place. Please discuss your individual situation with Louise Robinson.

Sometimes we like to capture special moments to share with our community. I agree that photos and videos of my child/me can be taken and used for Step Out! social media or promotional purposes. (IF YOU DO NOT WANT PHOTOS AND VIDEOS WITH YOUR CHILD TO BE UTILIZED ON THE INTERNET PLEASE TICK THE BOX AS REQUIRED, BUT ADD A COMMENT INDICATING THIS).
