Limited Space! Calling all 3-10 year old's! Back by popular demand! Get ready to break it down with hip hop moves and jurassic grooves in this earthshaking hop hop dino camp! As a member of DJ Dino’s Hip Hop Crew, your energetic dino dancer will explore the land of Hip-Hop-O-Toa and star in the annual Rappin’ Raptor Reptile Rock Out! Not only will he learn dino-mite hip hop moves (always age appropriate of course!), he’ll also dig the super-saurus crafts and dino games each day in this fast-paced, high energy camp! It’s sure to be a prehistoric time!
Camp will run Monday to Thursday from 9:00 - 12:00 daily (performance on last day). Early Bird Registration fee is $210 plus GST if you register before Feb 28, 2023. Registration will not be valid until full payment has been received.

PARENT/GUARDIAN AUTHORIZATION

I give permission for my child to participate in all “Little Groover” dance camp activities. I understand that there are potential risks associated with any program requiring physical activity, including this one. I hereby do declare my child to be physically sound, having medical approval to participate in the activities of the “Little Groovers” program.
I understand and agree that Dance City Inc, the host of “Little Groovers Dance Camp” does not assume any responsibility for medical expenses and/or compensation for any injury that my child may suffer during or resulting from participation in this program or any other activities at the “Little Groovers Dance Camp” location.
I agree that it is my responsibility to receive clearance from my child’s physician before permitting my child to participate in this or any physical activity. I further understand that neither Dance City Inc., “Little Groovers Dance Camp,” nor any of its paid staff or volunteer workers can be held responsible in the event of an accident. I certify that my child is amenable to discipline and free from habits or attitudes, which would make him/her an undesirable participant. I have studied this information and understand the contents thereof.

EMERGENCY AUTHORIZATION

I authorize any representative of Dance City Inc. to seek medical attention for my child when immediate medical care is warranted by the circumstances and I cannot be reached, or if under the circumstances there is no time to attempt to reach me because of the nature of the injury or illness. I further authorize the health care professional selected by Dance City Inc. to provide the necessary care and treatment for my child.

PAYMENT / CANCELLATION POLICY

Registration is not valid unless full payment has been received. Cancellations received before April 15, 2023 are eligible for a 100% camp fee credit for any future use. If cancellation is received between April 15 and 8 days prior to the camp start date, you will receive a 50% credit to use towards a future camp or intensive. Credit will NOT be issued if cancellation notice is received 7 days or less prior to camp start date. Cancellations are only accepted in writing through email to hello@reginadancecity.ca.

RELEASE AND WAIVER

In consideration of my child’s participation in the activities of “Little Groovers Dance Camp,” I do hereby agree to hold free from any and all liability Dance City Inc. – “Little Groovers Dance Camp” and its respective officers, employees and members and do hereby for myself, my heirs, executors and administrators, waive, release and forever discharge any and all rights and claims for damages which I may have or which hereinafter accrue to me arising out of or connected with my child’s participation in any of the activities of the “Little Groovers Dance Camp."

COMMUNICABLE ILLNESS AND COVID-10 WAIVER

I understand and acknowledge that participation in Dance City Inc., classes, programs, rehearsals or activities entails the risk of exposure to and infection/contraction of communicable illnesses, viruses and diseases, including, without limitation, SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) virus which can lead to the COVID-19 (collectively, a “Communicable Illness”). I voluntarily agree, therefore, to assume all risks and responsibility of contracting a Communicable Illness, which me or my child may contract during any of Dance City Inc. classes, programs, rehearsals, performances, or activities. I also exempt and release, Dance City Inc. and its owners, employees, contractors, assistants, volunteers, guest artists, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with any Communicable Illness.
I agree and give full consent to active symptom screenings for myself or my child which may include a temperature check. I understand and agree that I will not send my child to Dance City Inc. if they are sick even if symptoms resemble a mild cold. Symptoms to look for include, but are not limited to, fever, cough, shortness of breath, sore throat, runny nose, nasal congestion, headache, and a general feeling of being unwell.
I understand and have made my child aware that they are to inform a Dance City Inc. staff member immediately should any of the above-mentioned symptoms develop while at Dance City Inc. Should this occur I understand my child will be isolated until they are picked up by a parent or guardian. I understand it is my responsibility to ensure my child is picked up immediately when notified.
I understand and agree that I will adhere to any and all of Saskatchewan's Public Health Orders under The Pubic Health Act, 1994. Individuals who have a fever, cough, shortness of breath, sore throat, runny nose (that is not related to a pre-existing illness or health condition) and not permitted to enter the studio.
I have alerted Dance City Inc. of any and all medical conditions and allergies of my child. I understand it is especially important to alert Dance City Inc. of any health conditions my child has which may cause similar symptoms of SARS-CoV-2 and/or COVID-19. These health conditions and symptoms include, but are not limited to, a cough and/or shortness of breath due to asthma or exercise induced asthma, and/or a sore throat, runny nose and/or nasal congestion due to severe seasonal allergies.

Consumer Protection Act
Your Rights under The Consumer Protection Act. You may cancel this contract at any time during the period that ends Seven (7) business days after the latter of the day you receive a written copy of the contract and the day all the services are available. You do not need to give Dance City Inc. a reason for canceling during this period. In addition, there are other grounds that allow you to cancel this contract. You may also have other rights, duties, and remedies at law. For more information, you may contact the Consumer Protection Branch, Saskatchewan Justice. To cancel this contract you must give notice of cancellation to Dance City Inc., at 3889 Arcola Ave East, by personal service, by registered mail or by any other means set out in the contract.
