As a participant or participant's parent/legal guardian, I agree to assume all risks of any injury/injuries having arisen out of, connected with, or in any way associated with being present at the Next Stage Dance & Drama Academy, L.L.C.'s facility located at 651 Clements Bridge Road, Barrington, NJ. I do hereby fully release and discharge the Next Stage Dance & Drama Academy, L.L.C., its owners, employees, assistants, volunteers, guest artists and instructors from any and all claims from injury, damage or loss which I or my minor child may have or which may occur as a result of my participation in lessons held at the Next Stage Dance & Drama Academy. I fully understand that I or my minor child is not a student of the Next Stage Dance & Drama Academy, L.L.C. and is not being instructed by a Next Stage Dance & Drama Academy employee or contractor.
