The School of Ballet San Antonio is now accepting registrations for our Spring Intensive 2021! The intensive, 1-week training program, is designed for serious young ballet dancers who want to develop and grow as a dancer and artist. The program offers an array of classes led by professional instructors emphasizing ballet technique, pointe, variations, modern, and jazz.

Release and Waiver

The above-named individual is a participant (“Participant”) in the School of Ballet San Antonio (the “School” or “SBSA”). The additional undersigned are parent(s) and/or legal guardian(s) of Participant (collectively, “Parent”). Parent and Participant understand and agree as follows:

1. Assumption of Risk. On behalf of myself, my estate, my heirs and my representatives, I hereby voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by me, or any loss or damage of property owned by me, as a result of my activities.

2. Release and Indemnification. On behalf of myself, my estate, my heirs and my representatives, I hereby hold harmless, release and waive any and all liability, damage or claim of any nature against SBSA, its sponsors and affiliates, and any of their officers, agents, attorneys, employees or volunteers, that may arise out of or be related to my activities, including any illnesses related to COVID-19. Further, I agree to indemnify SBSA, its sponsors and affiliates, against all liabilities, claims of any nature, or damage that is brought by a third-party against SBSA, its sponsors and affiliates, based on my conduct.

3. No Insurance Coverage. I understand that, as a Participant, I am not provided with accident or medical insurance and SBSA, its sponsors and affiliates, are not responsible for any medical expenses I may incur as a result of an accident or injury. Further, I understand that I am not covered by Workers’ Compensation nor entitled to employee benefits.

4. Right to Photos, Images and Video. I hereby grant to SBSA, its sponsors and affiliates, the right to use and publish images, video, and any similar or related media depicting my participation in SBSA activities. This release shall extend to any and all use and publication of said photographs, images, or video, including electronic media. Publication includes by way of social media or any third-party websites.

5. COVID-19. I agree to comply with the applicable protocols mandated by SBSA during the COVID-19 outbreak or subsequent pandemic or public health crisis.

6. Force Majeure. SBSA reserves the right, in its sole discretion, to close the School due to pandemic health issues, civil disturbances, or for any other business reasons.

7. Understanding. I am aware of and understand that:
i. Ballet training requires sustained, repetitive, vigorous physical activity, usually performed on a hard or lightly padded surface without athletic shoes or other protective footwear;
ii. Dance instruction involves kinetic corrections that may include limited physical corrections of Participants as part of regular classwork and rehearsals;
iii. Participants engage in a broad range of quick movements, bending, twisting, running, leaping, and lifting, which place extreme demands on the human body, including stress of joints and ligaments, repetitive impact, and occasional falls, slips, and collisions with other Participants and objects;
iv. Participant may engage in physical training activities including stretching, weight lifting, and other exercises using gym equipment;
v. There is no guarantee that Participant will perform specific roles, obtain employment at a ballet or dance company, or otherwise progress in dance;
vi. SBSA is not staffed to monitor or supervise the activities of its Participants at all times;
vii. There are inherent risks of serious personal injury involved in these activities as well as in general participation in SBSA program, activities and events.

I further understand and agree to the terms and conditions of this agreement and I am signing this document of my own free will. Further, by signing this agreement, I attest to the fact that I am eighteen (18) years of age or older
