COVID-19 Symptom Check

The camper and his dropping off Parent/Guardian/Family Member understands that Liberty and other families benefiting from the Liberty Camps are relying upon them to strictly abide by these policies, and they further understand that failure to carefully and diligently abide hereby could have catastrophic health (if not fatal) ramifications for other Campers and/or their families. Parents and Campers will self check for symptoms by answering the screening card questions (COVID-1 Screening Card) each day before coming to camp and only come to camp if they pass the symptom checklist. Screening card will be sent to family prior to camp start date.

Underlying Medical Conditions

Campers with underlying medical conditions that make them more vulnerable to a COVID-19 or Corona Virus infection must not participate in the Liberty Camps without the written permission of a doctor. If you have any such conditions, please inform Liberty promptly, and provide Liberty with written permission from your doctor, as well as any special precautions that should be taken for your camper. Liberty reserves the right at all times to refuse to permit such an camper from participating in a Liberty Camp for any reason.

Group Size

Campers will be assigned to a specific group with a maximum of 10 students. The Campers in each group will be assigned a specific coach (or coaches in some cases) and the assigned coach will not work with any other Campers other than those in his group at any time. Groups are not allowed to commingle. Campers in their assigned group will maintain physical distancing when possible.

Drop Off / Pick Up

At this time all camps will be drop off and pick up only to reduce person to person interactions. Contact Event Director (TNuno@libertygymcenter.com) or Liberty Welcome Center (info@libertygymtraningcenter.com via email. Spot TV is being installed so parents can view camps from their mobile device remotely.

Staggered Start Times

Camps will be staggered so camp start times and end times are at least 15 minutes apart to ensure no overlap of groups. Campers will have a 10 minute window to arrive for camp (5 minutes before and 5 minutes after start time). Any camper that fails to enter the facility on a given day within 5 minutes after the scheduled start of his camp will not be allowed to enter that day. Campers that arrive earlier than 5 minutes before camp will be asked to stay in their car until coach gives thumbs up that facility is ready to allow Campers to enter. At the normal entrance of Building A, there will be a system or ropes (pathway) to guide Campers (with physical disctancing marks on the asphalt) that leads to an outside hand sanitation station. Campers enters the pathway (there will be a sign that says "ENTER") and proceeds to the next physical distancing spot on the asphalt. If no one is in line, the camper can proceed directly to the hand sanitation station and either washes hands with soap/water for 30 seconds or uses provided hand sanitizer. Early pick up must be communicated to director in writing prior to start of camp (Slack preferred).

Gym A Rooms

Gym A has been divided into "rooms". Only one group per room at a time. On the first day of camp for each group, there will be an orientation to explain how the new system works and go over the expectations for the Campers. Colored maps of the gym's rooms will be posted throughout the gym and red velcro will be placed on the carpet bonded foam where clear room identification is needed. Coach/Group of Campers are responsible for cleaning/sanitizing all equipment in their room following posted sanitiation procedures prior to departing from camp. Campers must clean hands (either with wet wipes, hand sanitizer or soap and water) at the end of every rotation within their room (Maintain physical distancing). Campers should seek at all times while in the room to maintain a distance of at least 6' from all other persons in their camp.

Bathroom Use

There are two bathrooms in Building A. Campers and coach(es) from the same group will be assigned a specific bathroom and the other group (if there is a second group) will be assigned the second bathroom to minimize the potential of cross contamination. After using the facility each user must follow the sanitizing procedure posted on the bathroom wall including washing hands with soap and water for a minimum of 20 seconds.

Physical Distancing

Liberty will follow reasonable cleaning/sanitation practices and follow orders/guidelines provided by CCC Health Department. Liberty Gymnastics will be operating in a physical distancing environment but even with the best efforts and intentions there will be times when the Campers and coaches will breach the 6' distancing recommendation. Our teaching and coaching staff will spot (physically assist) when the circumstances require it. Spotting our Campers is often necessary in order to teach skills safely, to help Campers perform skills correctly and to prevent injury. Initially, until county orders are clarified, coaches will wear masks when spotting or when physical distancing is not possible. I understand and agree that spotting will be part of the learning process at Liberty Gymnastics and I agree to permit my child's coach to physically assist my child when needed. Direct assistance will also be provided in the event of an injury.

Waiver

Release and Waiver of Liability Agreement

In consideration of participating in Liberty Gymnastics Training Center's recreational program I represent that I understand the nature of this Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity.

I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in the Activity.

I hereby release, discharge, and covenant not to sue Liberty Gymnastics Training Center, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "releasees" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.

I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extend allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

Waiver COVID-19

The camper and his/her parent and/or legal guardian (Parent/Guardian), the Camper's designated adult (family representative) acting on behalf of the family members, and the individuals residing at the same address as Camper, in exchange for (i) permitting Camper to participate in one or more 2020 Gymnastics Camp(s) operated by Liberty Gymnastics Training Center (Liberty) in calendar year 2020 (the 2020 Camps) at one or more of the Liberty facilities, and (ii) providing the Camper, Parent/Guardian, Family Representative, and/or Family Members with limited access to the Facilities as described in the Liberty Gymnastics COVID-19 Essential Business Operation Protocols, the Participants hereby on behalf of each Participant named and those Participants’ heirs, executors and administrators covenant not to sue any of the Releasees (consisting of Liberty, the owner of the Facilities (the Site Owner), any other participants in such 2020 Camps (including campers, their parents, guardians, family representatives or family members), and the respective owners, directors, officers, employees, agents, contractors, volunteers, associates, affiliates, or other event personnel whether volunteers or paid staff of any of the foregoing). In addition, the Participants knowingly and willingly release, waive, and discharge from any and all liability, claims, demands, damages, causes of action, losses, or expenses (including attorneys’ fees and expenses) to each Participant named herein, on account of physical, mental, or emotional injury, illness, or death of the person of Participant or to the property of Participant, whether such injury or death be caused by the negligence, or gross negligence of the Releasees or otherwise, suffered either during or as a result of the 2020 Camps or while traveling to or from the 2020 Camps (with all such claims hereinafter being referred to as the Released Claims). For clarity only, and without limiting the scope of the foregoing, the Released Claims also include therein any and all claims which do or may in the future arise out of and/or are related to injury, illness or death associated with or stemming from the known or expected contraction of COVID-19 or the Corona Virus as a result of the attendance at, or participation in, one or more such 2020 Camps by either Participant, Parent/Guardian, Family Representative, or any Family Member.

Waiver COVID-19 Continued

The Participants hereby acknowledge the following: (i) that each of the Liberty Facilities (Building A and B) are wide open, indoor gyms facilities that will each be divided up into as many as four (4) workout areas (designated “Rooms”) within which separate camps involving groups of gymnasts will be conducted; (ii) that these Rooms may not be physically separated by walls or any other physical divider, but may instead be separated (depending upon size and location) by no more than 10 feet of space marked off for that purpose; (iii) that air will flow freely between the Rooms in each Facility; (iv) that the gymnasts attending the 2020 Camps will be working out and doing gymnastics in close proximity with, and sharing equipment with, other gymnasts in each of the workout Rooms designated for such activity; (v) that the vast majority (if not all) of the persons and Participants associated with the 2020 Camps will be wearing NO FACE MASKS or any other facial coverings at or during the 2020 camps; and (vi) that while Liberty is taking the precautions described in the Protocols for the purpose of diminishing the likelihood of spreading COVID-19 or the Corona Virus between its gymnasts and other Participants, Liberty cannot guarantee persons participating in or associated with the 2020 Camps will not be exposed thereto as result thereof. Notwithstanding the foregoing, or any other provision of this Release and Waiver of Liability, Participants release the Releasees from any liability whatsoever arising from any injury, damage, illness or death to Participants during the 2020 Camps where said injury, damage, or death is the result of, or arises from any tortious or criminal conduct upon the part of a Liberty or the Site Owner (or their respective owners, directors, officers, employees, agents, contractors, volunteers, associates, affiliates, or other event personnel whether volunteers or paid staff). Participant hereby assumes full responsibility for, and risk of, bodily injury or illness to or death of Participants due to the negligence or gross negligence of Releasees. Participant also releases Liberty and the Site Owner, but no other Releasee, from any and all liability for the intentional or criminal acts of their respective owners, directors, officers, employees, agents, contractors, volunteers, associates, affiliates, or other event personnel whether volunteers or paid staff while Participant participates in or travels to and from the 2020 Camps.
Participant further expressly agrees that the foregoing release and waiver is intended to be as broad and inclusive as is permitted by the law of the State of California, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Participant has read and voluntarily signs this Release and Waiver of Liability, and further agrees that no oral representations, statement or inducement apart from the foregoing have been made, and that this Release and Waiver of Liability may only be modified by a written document signed by Participant and a duly authorized representative of the Liberty Gymnastics Training Center.

Video / Photographic Release

I understand that I have enrolled my child or children at Liberty Gymnastics Training Center located at 2330 Bates Ave (Suite A or B2/B3), also referred to herein as ”Liberty Gymnastics”. Liberty Gymnastics has a program whereby webcams are in use and my children are under streaming video surveillance that used for training and management purpose, in addition to, secure accessibility from the web in accordance with the terms and conditions associated with the Spot TV website (also referred to herein as “Spot TV”). By my signature below, I hereby consent to the photographing of myself and/or my minor child(ren) and the recording of my voice and or that of my child(ren) named above. I agree that I have no rights to any of the video footage or photographs for any reason at any time. I also agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from the Content and Service. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal. I assume full liability of all the terms outlined in this release for any family member’s account activity associated with my child or children. Copyrighted work includes all web streaming and video/audio recordings. I understand that legal action can be taken against me by Spot TV or Children’s Activity Center for such copy right infringement. I understand that the term "photograph" as used herein encompasses still photographs, audio, and motion picture footage.
