Edge Dance Center presents the Summer Performance for the 2019/2020 dance season at Burlington County Agricultural Center - Market Barn. Students must be registered for the event in order to participate.

As the legal parent or guardian, I release and hold harmless Edge Dance, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Edge Dance, its owners and operators or in route to or from any of said premises.

Related to Covid 19

Edge Dance Center (The Studio) has put in place preventative measures to reduce the spread of COVID-19; however, Edge cannot guarantee that you or your dancer/child(ren) will not become infected with COVID-19. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)s attendance at the Summer performance.

Health/Treatment Statement

I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. My child does not show any physical symptoms of illness and will not participate in activities at Edge Dance LLC if symptoms arise. The undersigned gives permission to Edge Dance, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.

This Performance will be recorded and photographed. Unless otherwise noted, Edge Dance has permission to use dance photographs and/or film for but not limited to studio premises, website, student instruction/education, social media marketing and/or business advertising.
