Photo Release Authorization

I understand that pictures may be taken before, during, and/or after classes, events, and shows. OCPAA has my permission to use my or my child's photograph publicly to promote the studio and its programs and events. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

General Release Authorization

I declare that I am 18 years of age or older or that I am registering myself on behalf of my child who is under 18 years of age (either party to be called, "I"). I am in good health and capable of undergoing the course of dance and/or exercise for which I am now enrolling.
I realize that I assume full responsibility and risk for any injury or damages which I might sustain either directly or indirectly from any cause whatsoever related to the course of dance, activity and/or exercise which I participate in at Orange County Performing Arts Academy - physically or virtually online. I also agree that the owner and operators of Orange County Performing Arts Academy shall not be responsible for any of my personal belongings or items or any loss or theft thereof. I understand clearly that no representations, warranties, or guarantees of any kind, whether expressed or implied, have been made to me regarding the results of the course of dance and/or exercise for which I am now enrolling.
I understand that I am not entitled to any refunds under any circumstances for services provided. I also understand that this course for which I am enrolling is non transferable.
I hereby release and forever discharge Orange County Performing Arts Academy, each and every owner, operator, employee, officer, partner, relative, assigns, and representative of Orange County Performing Arts Academy from any and all manner of action or actions, cause or causes of action, suits, debts, liens, contracts, agreements, promises, liabilities, claims, rights, obligations, damages, losses, and expenses of any nature whatsoever, known or unknown, which I now have or may hereafter have against each or any of the above persons or entities by reason of my use of the facilities of Orange County Performing Arts Academy or my participation in any course of dance and/or exercise at Orange County Performing Arts Academy. By releasing and forever discharging claims both known and unknown, I expressly waive any rights I have under California civil code section 15-22 which provides: "a general release does not extend to claims which the creditor does not know or suspect to exist in his favor at the time of executing this release, which if known by him must have materially affected his settlement with the debtor, as well as any other statutes and/or common law principles or similar effect arising in any jurisdiction." This release is binding upon me and my heirs, successors, and assigns.
I acknowledge that I have carefully read this entire document before signing it. I fully realize that this is a binding legal document which affects any claims I might have had resulting from any use of the facilities of Orange County Performing Arts Academy and participation in any at studio or online courses offered by Orange County Performing Arts Academy.
