Times: 1:30-2:00pm 7-8 year Registration 2:00-2:30pm 7-8 year Audition Attire: Ladies: Black leotard, pink tights, ballet slippers, ballet bun Gentlemen: Black shirt, black athletic pants or black tights, black ballet slippers All dancers are welcome, any studio affiliation, must take a minimum of one (1) ballet class per week. Company Membership Fees will be charged once dancers have been casted. Scholarships & Financial Aid available upon request. Age as of January 1, 2020. Even if you have pre-registered online you must attend the studio registration prior to the audition to receive your audition number as well as important info. Audition fees in the amount of $40 are due prior to audition date or at registration. Payable by cash or check to UFAA Only.

I hereby consent to participate or have my child participate in the activities offered at the facility owned by Rock City Properties LLC (RCP) in conjunction with United Talent Inc. (UT) and United Fine Arts Academy (UFAA). The risks involved with respect to such a program are fully understood. It is hereby agreed that I/my executors, waive and release all rights and claims for damages that I may have at any time against RCP, UT or UFAA and their representatives whether paid or volunteer, for any injury, permanent disability, or death in connection with all of RCP's and UFAA's programs, or other activities related to RCP, UT or UFAA. I certify that I have medical insurance that will provide coverage while I or my child is participating in activities at RCP, UT or UFAA. I confirm that I and or my child are in good health. I hereby authorize simple first aid and consent to any x-ray, exam, and medical or surgical diagnosis, which are deemed necessary in case of emergency.
