As the legal parent or guardian, I release and hold harmless GYMNASTICS WORLD, LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of GYMNASTICS WORLD, LLC, its owners and operators or in route to or from any of said premises. The undersigned gives permission to GYMNASTICS WORLD, LLC, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the nearest hospital. Please include physicians' phone number under your account. All Gymnastics World, LLC programs are designed for a minimum of two times per week attendance. All students of Gymnastics World are required to enroll in two class per week for a minimum of one class on two different days of the week. Minimum Tuition Fees: $160.00 per month. This will be charged as soon as you submit your online registration and is non-refundable (even if you decide not to come to any classes). If you are registering your child/children for the first time in any of our programs, you are subjected to a non-refundable registration fee of $80.00/student at the time of your registration. All families will be under auto draft payments method (with choice of draft on the 1st or the 15th day of the month). Tuition will be drafted now and then on your billing date thereafter. Tuition is due prior to the month of service. If you wish not to be drafted, please pay your tuition prior your billing date by paying in person, over the phone, or online. If accounts are paid 10 days after of your billing date, there will be a $25.00 late fee applied to the account balance. There is a $35.00 returned check charge for any checks returned by our bank. ***IMPORTANT INFORMATION - PLEASE READ*** In case you need to withdraw you child/children from our programs for a set period of time, you must fill out a freeze form 10 working days prior your next billing date to avoid any future charge to your account. Freeze account policy: A membership can be frozen no more than 3 months within a year unless approved by management. There is no fee for freezing. However, in order to hold your child's current class spot, you must pay for the month that your child returns prior to freezing your account (This amount is non-refundable if you decide not to return). Because you are freezing and holding your child's current classes, your child is still a student of Gymnastics World and will not have to pay the registration fee when returning. Once you submit your registration and wish to terminate your account (cancel your membership), you are required to complete a cancellation form and turn it back into the Gymnastics World's office/front desk a cancellation form 10 working days prior your next billing date to avoid any further charges to your account. The 10 working days notice rule will not be waived in any case. I, the parent/legal guardian of the student(s) listed under my account am fully aware that he/she will be engaging in physical exercise and that the use of exercise equipment, the club facility, training and instruction could cause injury. I am voluntarily allowing him.her to participate in these activities and assume responsibility for any risk and /or injury that may result. I agree to waive any claims or rights i might otherwise have to sue the gym, its owners, employees or other related agent. I also understand that she/he will conduct herself/himself in an appropriate manner. Failure in comply with the rules and regulations
