Dance 101 is offering a Parents' Night Out Dance Camp on either Friday, April 8th or Saturday, April 9th from 4-7pm for our Aloha! Dance Camp. The Aloha! Dance Camp will combine beach party fun with jazz dance steps. Parents will get 3 hours of alone time to plan their summer while their children can enjoy dancing to their favorite beach tunes! Participants will take a themed jazz class, make a craft, and enjoy dinner at the Dance 101 Aloha! Dance Camp! Aloha! Dance Camp is open to dancers ages 3-12 from all backgrounds! Cost is $45 per student. Students will need to bring a sack dinner.

Liability Waiver

I understand that dance and exercise programs require physical exertion and that physical injury may result from participating in such activities. Despite this risk, I am voluntarily participating in such activities and I voluntarily elect to assume all risk involved.
In consideration of my participation, I release and hold harmless Emily Finch, LLC, doing business as Dance 101 ("Dance 101"), its owners, administrators and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Dance 101, its owners, administrators, and instructors or in route to or from any of said premises. Further, I release and hold harmless Dance 101, its owners, administrators and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by any non-student/non-participant(s) who may accompany me, while in or upon the premises or any premises under the control and supervision of Dance 101, its owners, administrators, and instructors or in route to or from any of said premises.

Medical Emergency and Physical Condition

I hereby certify that participant is in proper physical and mental condition to participate in the enrolled activities. I agree to notify Dance 101 immediately and in writing if there is any change in participant's condition. I have disclosed any and all physical or mental issues, restrictions, or conditions regarding the participant on the Dance 101 registration form. I give Emily Finch, LLC, doing business as Dance 101 (”Dance 101”), its owners, administrators and teachers permission to seek emergency medical treatment for participant and I request that participant be transported to the nearest hospital if necessary. I understand that Dance 101 cannot dispense any form of medication.

Covid-19 Policy

I understand that this release also covers liability, claims, and actions caused entirely or in part by Dance 101's failure to follow the safety precautions or its failure to take additional precautions, including but not limited to negligence, mistake, or failure to enforce or supervise. I also assure Dance 101 that I will abide by their protocol, including not entering the premises if I have a fever or other flu-like symptoms. I further assure Dance 101 that I have adequate health, property, and liability insurance necessary to provide for and pay any medical costs or property damage that may directly or indirectly result from my presence at Dance 101. I will also indemnify and hold Dance 101 harmless for any damages I may cause. The recommend protocols and risks for COVID-19 are constantly changing. Dance 101 has no reliable way to stay fully updated on all of this information. Nor is it practicable or reasonable to fully implement every possible safety precaution.

Photo/Video Release

I understand that from time to time Emily Finch, LLC, doing business as Dance 101 (“Dance 101”) participants will be photographed and/or videotaped while participating in any Dance 101 activity. I further understand that these photos and videos may be used for appropriate promotional purposes for Dance 101 or may be provided to, or purchased by, participant's friends and family to be kept as treasured mementos. I hereby authorize Dance 101 and any Dance 101 hired photographers and videographers to take photographs of and videotape participant at recitals and other similar types of performances, including practices for such performances. I hereby waive any right to inspect or approve any image or use and grant Dance 101 permission to copyright such photographs and video in its name. Participants' last names will not be used on the Dance 101 website or in Dance 101 print advertising.

Instructor Feedback and Correction

I understand that in the instruction of dance the instructor may need to physically manipulate the participant when assisting the participant in the proper alignment and execution of the movements. I hereby authorize the instructors to physically manipulate the participant solely in order to assist in the proper alignment and execution of the movements.

I UNDERSTAND AND CONFIRM THAT BY SIGNING THIS LIABILITY WAIVER THAT I AM GIVING UP SUBSTANTIAL FUTURE LEGAL RIGHTS THAT I MAY HAVE ON BEHALF OF MYSELF, MY CHILD OR MY WARD TO BRING A LEGAL ACTION OR ASSERT A CLAIM FOR INJURY OR LOSS AGAINST EMILY FINCH, LLC, doing business as DANCE 101.
