Preschool Boogie Camp - Looking for a week of fun, dance, and more? Look no further, ROOTS Preschool Dance camp is the perfect opportunity for your child (ages 3-5 years) to experience different styles of dance including ballet, jazz, tap and more! Everyday will have a different craft, games and more fun! Must be potty trained to attend camp. Come Boogie with us!! Monday - Friday 9 am - 11 am. Dress Code: Dancers will need a solid-colored leotard, pink tights, and pink leather ballet shoes. Hair needs to be pulled up and out of the face. Register before March 31 to receive the Early Bird Price of $135.

By the electronic signature of us, the Child/Participant and the Parent or Legal Guardian (hereinafter collectively referred "Participant") agree as follows:

We understand that the School of Worship, LLC d/b/a/ ROOTS Academy ("RA") does not give credit and/or refunds for class(es) missed due to holidays, vacations, illness, weather, removal from participation in competition teams, etc. Once a payment has been made, RA does not provide refunds.

We understand that with an intense physical activity and that injuries will happen. Like all sports, people get hurt, sometimes very seriously.

Participant desires to voluntarily participate in the RA program. Participant acknowledges and agrees that there is a substantial risk of injury. Participant agrees to accept and assume all risks, including, but not limited to, all injuries, damages, costs of treatment, long term and lifetime effects of injury, disabilities, and other inconvenience and costs, arising from any injury.

During the second quarter of 2020, a worldwide pandemic was declared as a result of the coronavirus (COVID-19). The Center For Disease Control (CDC) states that "the virus that causes COVID-19 is thought to spread mainly from person to person, mainly through respiratory droplets produced when an infected person coughs or sneezes." As the CDC learned more about the virus, they alerted the public on possible preventive measures. RA will continue to follow updates from the CDC, our state and local standards of conduct and will put in place reasonable preventative measures to reduce the spread of COVID-19 at the academy. However, even though such standards will be followed and reasonable measures put into place, RA cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the academy could increase your risk and your child(ren)'s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and that my child(ren) and I may be exposed to or infected by COVID-19 by participating in RA activities (whether on-site or off-site) and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at RA may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees of RA, volunteers, and program participants and their families.

Considering the foregoing, however, I grant permission for my child(ren) to participate in all academy related activities, whether on or off-site, notwithstanding the risks associated with the COVID-19 virus and group activities.

I further agree on behalf of myself, my child named herein, and my spouse, our heirs, successors, and assigns, to release, indemnify, hold harmless, and defend RA and its members, directors, officers, employees, agents and representatives (''indemnitees") associated with the event arising from or in connection with the negligent acts or omissions of the indemnitees' in relation to prevention of the spread of the COVID- 19 virus. I SPECIFICALLY ACKNOWLEDGE AND AGREE THAT I AM AGREEING TO DEFEND, INDEMNIFY AND HOLD HARMLESS THE INDEMNITEES' FROM THEIR OWN NEGLIGENCE IN REGARD TO THE INDEMNITEES' NEGLIGENT ACTION AND/OR INACTION IN REGARD TO PROTECTION AGAINST THE COVID-19 VIRUS.

Participant voluntarily agrees to assume all of the foregoing risks and accept sole and exclusive responsibility for any injury to Participant (including your Participating child(ren) and/or or yourself). "Injury" includes, but is not limited to, personal injury, property damage, disability, death, illness, damage, loss, claim, liability, or expense, of any kind, that you or your child(ren) may experience or incur in connection with your child(ren)'s attendance at RA or participation in RA classes and or programs.

The Participant hereby releases, covenants not to sue
