Welcome to Liberty Gymnastics' Recreational Winter Camp!
Campers will enjoy working on new gymnastics skills, playing games, and making new friends! Campers will also be crafting gifts to bring home to their families.
$220 for 4 day Camp from 9AM to 2 PM. Dates: Monday, 12/28/20, Tuesday, 12/29, Wednesday 12/30 and Thursday 12/31/20. No rerunds after 12/19 and Deadline for enrolling 12/23

Release and Waiver of Liability Agreement
In consideration of participating in Liberty Gymnastics Training Center's recreational program I represent that I understand the nature of this Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity.
I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in the Activity.
I hereby release, discharge, and covenant not to sue Liberty Gymnastics Training Center, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extend allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

I understand that I have registered my child or children for recreational programs, camps, and/or events at Liberty Gymnastics Training Center, Inc., also referred to herein as Liberty Gymnastics, located at 2330 Bates Ave (Suites A and B2/B3), Concord, California 94520. Liberty Gymnastics has a program whereby webcams are in use and my children are under streaming video surveillance that is used for training and management purposes, in addition to secure accessibility from the web in accordance with the terms and conditions associated with the Spot TV website, also referred to herein as Spot TV. I hereby consent to the photographing of myself and/or my minor child(ren) and the recording of my voice and/or that of my minor child(ren) when we are within the Liberty Gymnastics facilities. I agree that these photographs and/or audio and/or video recordings singularly or in conjunction with other photographs and/or recordings may be used for internal training, management of the facility, advertising, publicity, commercial or other business purposes. I further agree that the video footage is the sole property of Liberty Gymnastics and may only be used by Liberty Gymnastics. I agree that I have no rights to any of the video footage or photographs for any reason at any time. I also agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from the Content and Service. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal. Copyrighted work includes all web streaming and video/audio recordings. I understand legal action can be taken against me by Spot TV or Liberty Gymnastics for such copyright infringement. I understand that the term photograph as used herein encompasses both still photographs and motion picture footage. I assume full liability of all the terms outlined in this release for any family member's account activity associated with my child(ren).

Liberty Gymnastics has modified its operation, facilities, and protocols (Protocols) to meet or exceed Contra Costa County health guidelines relating to COVID-19 and to create safe environments for staff and students/customers. The health and wellness of your child and our staff is our number one priority. Below are more details about Liberty Gymnastics' operations, protocols, and expectations of our customers to prevent the spread of COVID-19. These operations, protocols, and/or expectations may change when new guidelines are released.
At this time all classes will be drop-off and pick-up only to reduce person to person interactions. Observing classes from within our facilities, Buildings A and B2/B3 (also known as Gym A and Gym B, respectively), are currently suspended. At the normal entrance of Gym A and Gym B, there are social distancing marks on the ground and a hand washing station for use before entering and after leaving our facilities. Children arriving for class must maintain social distancing. All children must wash hands with soap and water for 30 seconds prior to entering the facility through the main double door entrance. Parents may assist children to wash their hands or enter the facility and the coach will be at the entrance to gather the students. Recreational students (ages 6 ) leave the gym through the main double door entrance/exit. Liberty Gymnastics has contracted with SpotTV to allow parents to watch their children remotely from their personal mobile device(s) or computer(s) as a complimentary service. Go to SpotTV.pro to register for Spot TV. Access to Spot TV will be available within 48 hours after completing Spot TV's registration and Liberty Gymnastics' class registration. NOTE: Streaming access is only available during your own children's enrolled class times or Camps.

Face Masks / Social Distancing / Spotting / Clean & Sanitize

Staff will wear masks at all times in common areas and on the gym floor.
Students will maintain a distance of at least 6' from all other persons in their class. Liberty Gymnastics will be operating in a social distancing environment but even with the best efforts and intentions there will be times when the students and coaches will breach the 6' distancing recommendation.

COVID-19 Release and Waiver of Liability

The Student and his/her parent and/or legal guardian (Parent/Guardian), assuming the role of the Student's designated adult family representative (Family Representative), acting on behalf of the student's family members and the individuals residing at the same address as the student (all the foregoing jointly referred to herein as Household Members), in exchange for (i) permitting the student to participate in one or more 2020 Liberty gymnastics programs including camps, classes, and/or events operated by Liberty Gymnastics Training Center, Inc. (herein referred to as Liberty) in calendar year 2020 at one or more of the Liberty facilities (Facilities), and (ii) providing the student, Parent/Guardian, Family Representative, and/or Household Members with limited access to the Facilities as described in the Liberty Gymnastics COVID-19 Business Operation Protocols (Protocols), hereby on behalf of each Household Member and those Household Members' heirs, executors and administrators covenant not to sue any of the Releasees (consisting of Liberty, the owner of the Facilities (Site Owner), any other participants in such 2020 Liberty gymnastics programs (including but not limited to students, their parents, guardians, family representatives or family members), and the respective owners, directors, officers, employees, agents, contractors, volunteers, associates, affiliates, or other event personnel whether volunteers or paid staff of any of the foregoing). In addition, the Household Members knowingly and willingly release, waive, and discharge from any and all liability, claims, demands, damages, causes of action, losses, or expenses (including attorneys' fees and expenses) to each Household Member, on account of physical, mental, or emotional injury, illness, or death of the person of Household Member or to the property of Household Member, whether such injury or death be caused by the negligence, or gross negligence of the Releasees or otherwise, suffered either during or as a result of the 2020 Liberty gymnastics programs or while traveling to or from the 2020 Liberty gymnastics programs (with all such claims hereinafter being referred to as the Released Claims). For clarity only, and without limiting the scope of the foregoing, the Released Claims also include therein any and all claims which do or may in the future arise out of and/or are related to injury, illness or death associated with or stemming from the known or expected contraction of COVID-19 or the coronavirus as a result of the attendance at, or participation in, one or more such 2020 Liberty gymnastics programs by either student, Parent/Guardian, Family Representative, Family Member, or any Household Member.
I, the legal parent/guardian of my child, a student of Liberty Gymnastics, and designated Family Representative acting on behalf of Household Members, hereby acknowledge the following: (i) that each of the Liberty Facilities (Buildings A and B2/B3) are wide open, indoor gym facilities that will each be divided up into multiple workout areas (designated Rooms) within which separate classes involving groups of students will be conducted; (ii) that these Rooms may not be physically separated by walls or any other physical divider, but may instead be separated (depending upon size and location) by no more than 10 feet of space marked off for that purpose; (iii) that air will flow freely between the Rooms in each Facility; (iv) that the students attending the 2020 Liberty gymnastics programs will be working out and doing gymnastics in close proximity with, and sharing equipment with, other students in each of the workout Rooms designated for such activity; (v) that the vast majority (if not all) of the students associated with the 2020 Liberty gymnastics programs will be wearing NO FACE MASKS or any other facial coverings; and (vi) that while Liberty is taking the precautions described in the Protocols (outlined in previous sections) for the purpose of diminishing
