Students must be registered for the CPYB 2018-19 Academic Year before submitting this form requesting scholarship. Families requesting scholarship should email an electronic copy of federal income tax forms (1040, 1040A or 1040EZ, etc) and three required photos via e-mail to plummer@cpyb.org. Scholarship applications are NOT considered complete without federal income tax forms and photos submitted*.
PLEASE NOTE: Children's Division students cannot apply for scholarship.
*Students that attended the 2017-18 Academic Year, 2018 5-Week Summer Ballet Program or 2018 August Course do NOT need to submit photos.*

SCHOLARSHIP APPLICATION AGREEMENTS

1. I understand Changing Lives Scholarship awards are for students who will be attending the entire CPYB 2018-2019 Academic Year.
1a. I understand that if I have requested to be considered for a 2019 5-Week Summer Ballet Program under this application, an award is not guaranteed immediately.
2. I understand that my student will not be reviewed for scholarship unless all required materials are completed in its entirety.
3. I understand CPYB does not keep your electronic tax information after the scholarship review process. All documentation is permanently deleted.
4. I understand students receiving scholarship aid accept the responsibility of performing age-appropriate duties around the studios, assigned by CPYB.
5. I understand that scholarship students are required to sign a mutual agreement upon acceptance of the scholarship. In this agreement, the student agrees to attend the required minimum number of classes at their level, to follow the school rules as outlined in the student handbook on CPYB.org (including Private Class Policy), and to perform the assigned tasks in a satisfactory manner.
