Release of Liability
I hereby acknowledge and agree to the following in consideration of being permitted to participate in dance and theater activities at Stage Door Arts Academy.
I understand that participation in dance and theater activities inherently involves certain risks, including but not limited to, physical exertion, potential injury, and unforeseen accidents. I voluntarily assume all risks associated with participation in classes, rehearsals, and performances.
I release Stage Door Arts Academy, its instructors, employees, partners, and affiliates from any and all liability for personal injury, property damage, or wrongful death resulting from such participation. Additionally, I agree to indemnify and hold harmless Stage Door Arts Academy, its instructors, employees, partners, and affiliates from any claims, damages, liabilities, costs, or expenses arising out of or related to my participation in dance and theater activities.
I have read and understood this release of liability and voluntarily agreed to it with full knowledge of its significance.
