Join us for a Summer themed night of gymnastics, pizza, and crafts! For ages 5-10 and cost is $35 per child. Must be able to participate independently. Must have a minimum of 5 participants to have the event. No refunds, transfers, prorating, or make-ups allowed. Declined payments will result in your child being dropped from this event. **Doors open 10 minutes before event starts**

AGREEMENT OF RISK & WAIVER OF LIABILITY POLICY
I/we give permission for my child and myself to participate in classes/activities at Missouri Gymnastics Academy LLC (MGA) under the terms/conditions set out below.
• I understand the nature of this activity and that my child/I am qualified and in good health and in proper physical condition to participate independently in such activity.
• I fully acknowledge, understand, appreciate and agree, that this activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my/my child's own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the Releasees named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I/my child incurs as a result of my participation in the activity.
• I further acknowledge, understand, appreciate and agree that my/my child's participation may result in possible exposure to an illness from infectious diseases. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.
• I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and I assume full responsibility for my/my child's participation and exposure.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, and I understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete an unconditional release of all liability to the greatest extent allowed by law.
I agree that if any portion of this Agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect. To the extent permissible by law, I/we hereby release, discharge and/or otherwise hold harmless and indemnify MGA, its owners, officers, directors, employees and associated personnel, from and against any and all demands, claims and causes of action arising, directly or indirectly, from my or my child's/ward's participation in its programs.
THIS RELEASE SPECIFICALLY INCLUDES ANY DEMANDS, CLAIMS AND CAUSES OF ACTION ARISING OUT OF THE PAST OR FUTURE NEGLIGENT ACTS AND/OR OMISSIONS OF MGA, ITS OWNERS, EMPLOYEES AND ASSOCIATED PERSONNEL.
AGREEMENT OF MEDICAL AUTHORIZATION
First aid will be administered for minor injuries. In the event of a major injury, I/We give permission for MGA owners, coaches, and/or employees, to seek emergency medical treatment for the participant(s) in the event they are unable to reach any parent/guardian. MGA will contact parents/legal guardians listed on the Parent Portal. Please list an additional contact in the event we cannot reach a guardian.The undersigned also agrees that they themselves will be responsible for any financial debt incurred by said action. MGA is not responsible for any fees associated with transporting or treatment of the injured child. All fees are the responsibility of the parents/legal guardians.

AGREEMENT OF MARKETING RELEASE
Occasionally MGA uses photos or videos of its students in ads, on its website, or other marketing mediums. I give consent to MGA to use any pictures and/or videos of my child(ren). No personal information will be used or given out. MGA will not sell any photos and/or videos to any outside companies, nor will any personal information be used or given out. These photos and/or videos remain the property of MGA
Parents/legal guardians, adults, or children CANNOT take any pictures and/or videos other than their own gymnast or self. Anyone found to be taking pictures or videos without permission will be warned and possibly terminated from all classes, team, and activities at MGA.
