- Age Requirement: Open Jams is open to children aged 7 and up.
- Free Workout: This is an unstructured gym time; no structured classes will be offered.

I understand that participation in Open Jams involves physical activity, which carries a risk of injury. I acknowledge that my child is voluntarily participating in Open Jams and assume all risks associated with this participation.

I hereby release, waive, discharge, and covenant not to sue The Jam UAC, its owners, employees, agents, and representatives (collectively, the "Releasees") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, or to any property belonging to me or my child, while participating in Open Jams.

I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my child’s participation in Open Jams, whether caused by the negligence of the Releasees or otherwise.

I certify that my child is in good health and physically able to participate in Open Jams. I will inform the gym staff of any health conditions or special needs that my child may have.

In the event of an emergency, I authorize the gym staff to obtain medical treatment for my child. I understand that I will be notified as soon as possible and that I am responsible for any medical expenses incurred.

I understand that my child is expected to follow the gym’s rules and behave respectfully towards other participants and staff. I acknowledge that disruptive or unsafe behavior may result in my child being asked to leave Open Jams.

I consent to my child being photographed and/or videotaped during Open Jams. I understand that these images and videos may be used for promotional purposes, including social media, websites, and other marketing materials.
