I/we hereby give consent and authorization for the owner, directors, employees, instructors, coaches or agents of Sunbelt gymnastics Academy, Inc. {hereafter "supervising adult} to administer general first aid treatment for any minor injuries or illnesses experienced by my/our child.

I/we covenant not to sue Sunbelt Gymnastics Academy, Inc., its owner, directors, employees, instructors, coaches, or agents for any loss or damages and/or injury to my/our child while participating in activities at Sunbelt Gymnastics Academy, Inc.

As the parent{s} or legal guardian{s}, I hereby agree to be solely responsible for possible future medical expenses which may be incurred on behalf of my/our child as a result of any injury sustained while training or under the direction of Sunbelt Gymnastics Academy, Inc., its owner, directors, employees, instructors, coaches, or agents.

This acknowledgement of risk and waiver of liability, having been thoroughly read and understood completely, is signed and accepted voluntarily as to its content and intent.

I/we hereby give consent and authorization to use by Sunbelt Gymnastics Academy, Inc. of a photo{s} {likeness} of my/our child in any and all of its promotions and advertising materials and agree that Sunbelt Gymnastics Academy, Inc. may also film my/our child while h/she is participating in gymnastics class activities, all without compensation.

As the parent{s} or legal guardian{s} of the enrolled child, I {we} hereby give consent for him/her to participate in the programs offered at Sunbelt Gymnastics Academy, Inc. I/we recognize that potentially severe injuries can occur in any activity involving height or motion, including gymnastics. I/we fully understand and accept this risk. I/we realize that my/our child will be performing and training on all gymnastics apparatus plus other training devices including, but not limited to, the trampoline, rock wall and inflatable devises.

Therefore, in consideration of Sunbelt Gymnastics Academy, Inc., allowing my/our child to use the equipment and facilities and allowing them to participate in Sunbelt Gymnastics' programs and activities I/we hereby forever release Sunbelt Gymnastics Academy, Inc. its owner, directors, employees, instructors, coaches, and agents from all liability for any and all damages and /or injuries {no matter the reason or cause} suffered by my/our child while under the supervision, instruction or control of Sunbelt Gymnastics' owner, directors, employees, instructors, coaches, and agents.
