I understand and acknowledge that in this release, (Assumption of Risk and Waiver of Liability) that N10 Dance Studios shall not be responsible for any injury or accident suffered by the student. This waiver is in protection of N10 Dance Studios owners, faculty, instructors, guest instructors, employees, agents, representatives, groups or individuals affiliated therewith (each a ”Released Party” and collectively, the “Released Parties”) on or off property.

This release extends to every claim, demand or liability of any kind, based on any injury, accident, or incident, foreseeable and unforeseeable, as a result of participating in dance classes, activities, competitions, programs or instruction (the “Activities”).

I understand and acknowledge that all activities conducted at and related to N10 Dance Studios have inherent risks of injury. I assume all known and unknown, foreseeable and unforeseeable risks of participating in the Activities, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the Released Parties. When undertaking the Activities, I recognize that the child/student is exposing himself/herself/themself to many risks, including but not limited to, muscle fatigue, muscle strain, broken bones, headaches, dizziness, and mental or emotional distress. We, parent/guardian and child/student, represent that we are voluntarily participating in the Activities and assume and accept all risks involved for such participation.

I understand and acknowledge that N10 Dance Studios is not responsible for my child/student or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio and that N10 Dance Studios will only be supervising my child/student when he or she is participating in the Activities.

I understand and acknowledge that the Released Parties are not responsible for any personal property loss or damage at any location. I understand that bringing any personal and valuable items are at my own risk. I understand that I have been advised to and will obtain such insurance or have agreed to take responsibility for any costs for loss or damages on my own.

I voluntarily assume all the foregoing risks and accept sole responsibility for any injury to the child/student (including but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation in the Activities (“Claims”).

I, intending to be legally bound for myself and my heirs, personal representatives, next of kin, and anyone who might make a claim on my behalf (each a “Releasing Party”), hereby voluntarily waive, release, discharge, holding harmless and covenant not to sue the Released Parties and collect from any and all Claims, including Claims of any kind for damages and/or liability, whether caused by the actions, omissions, or negligence of a Released Party, or otherwise related to my child/student participation in the Activities and promise not to sue the Released Parties for any damages incurred in connection with the Activities. This release extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown.

I have FULLY READ, understand the terms of the N10 Dance Studios Assumption of Risk and Waiver of Liability Form, and agree to all the terms above.

I expressly agree that this release is intended to be as broad and inclusive as the State of California will allow and that if any portion is held invalid, I agree that the balance shall, notwithstanding, continue in full legal force and effect.

I acknowledge that I am providing an electronic signature in lieu of an original signature on paper and subject to the electronic signature consent that I have provided separately.

Due to the outbreak of the novel Coronavirus (COVID-19), N10 Dance Studios is taking extra precautions with the care of every dancer to include enhanced sanitation/disinfection procedures in accordance with the Orange County Department of Health.

I am giving N10 Dance Studios consent to take photos, videos, and voice recordings at any time. I grant you permission to use my name, photos, videos, and my material about me for any marketing, promotional publicity for our program. I understand that N10 Dance Studios faculty and staff have the rights to all images and videography taken at the studio, studio events/performances off-site or for any projects to support our mission as a company.
