By registering my child for the Elevate Sports Center Spring Recital, I understand and agree that the $79 fee is non-refundable and is required at the time of registration. This fee secures my child’s spot in the recital and will not be refunded under any circumstances, including withdrawal from the recital for any reason. I acknowledge that this policy is in place to cover administrative costs and recital preparation expenses.

I understand that by registering my child for the Elevate Sports Center Spring Recital, I am granting permission for my child to be photographed and/or recorded during the event. Due to the nature of the recital, photos and videos will be taken as part of the production, and my child may appear in these materials. I acknowledge that Elevate Sports Center may use these photos or videos for promotional purposes, including but not limited to social media, advertising, and website content. I understand that this media release is a requirement for participation in the recital and that no compensation will be provided for the use of these materials.

I acknowledge that participation in the Spring Recital involves physical activity, and I hereby release Elevate Sports Center, its employees, and affiliates from any liability for injuries, accidents, or unforeseen incidents that may occur before, during, or after the event. I confirm that my child is in good health and able to participate safely. I accept full responsibility for any medical expenses that may arise due to my child’s participation in the recital.

It is the sole responsibility of the parent or guardian to ensure their child is properly sized for their recital leotard. Elevate Sports Center will arrange a day to have children sized in class, however if my child misses this class, I understand that it is my responsibility to make an appointment to bring my child in to get sized, or provide my child's sizing based on the measurement charts provided by the leotard manufacturer. Elevate Sports Center is not responsible for any sizing errors due to incorrect measurements provided by the parent or guardian. All leotard sales are final, and no refunds will be issued for incorrect sizing. Exchanges are not guaranteed and are subject to availability of alternate sizes. If an exchange is possible, additional costs may apply.
