Assumption of Risk
(Show-Hide Details)
1.Purpose:
Participants, parents, and legal guardians acknowledge that cheerleading, like any physical activity, carries inherent risks. These risks may include, but are not limited to, sprains, strains, fractures, concussions, and other potential injuries.
2.Policy:
2.1. Voluntary Participation:
By enrolling in the Sherwood Youth Cheerleading program, participants and their legal guardians voluntarily assume all risks associated with participation in cheerleading activities. Participants understand that they have the option to withdraw from any activity if they feel uncomfortable or unsafe.
2.2. Health and Fitness Responsibility:
It is the responsibility of participants and their legal guardians to ensure that they are physically and mentally fit to participate in cheerleading activities. Participants should inform program instructors of any pre-existing medical conditions or injuries that may affect their ability to safely engage in the program.
2.3. Supervision and Instruction:
The Sherwood Youth Cheerleading program will provide qualified instructors who will supervise and guide participants during activities. However, participants and their legal guardians understand that the presence of instructors does not eliminate the inherent risks associated with cheerleading.
2.4. Safety Guidelines and Equipment:
Participants are expected to adhere to all safety guidelines and instructions provided by program instructors. This includes wearing appropriate safety gear and using equipment properly. Failure to comply with safety guidelines may result in dismissal from the program.
2.5 Emergency Procedures:
In the event of an injury or emergency, program staff will provide appropriate first aid and seek medical attention if necessary. Participants and their legal guardians consent to emergency medical treatment if deemed necessary by program staff.
2.6. Release of Liability:
Participants, parents, and legal guardians hereby release and indemnify Sherwood Youth Cheerleading, its organizers, instructors, volunteers, and affiliated entities from any and all liability for injuries, damages, or losses sustained during participation in the program, to the fullest extent permitted by law. Agreement of this policy revokes the right of the parent/guardian of the cheerleaders from recovering damages in any event of injury or death.
2.7. Review and Understanding:
By signing this assumption of risk policy, participants, parents, and legal guardians acknowledge that they have read, understood, and agree to abide by the terms and conditions outlined herein. Parents and guardians enter into this agreement freely and voluntarily and agree that it will be binding upon them, their heirs, and legal representatives.
I've read the above and agree.
