From time to time members of the media and/or staff of Elite Dance Center may take photographs and/or video of the dance students for use in presentations, publications and/or websites and to promote Elite Dance Center. Whenever possible we will give you notice prior to photographs being taken, however please be advised that this is not always possible. I understand that these images may be edit- ed, duplicated, reproduced and/or reformatted by Elite Dance Center without payment of fees, in perpetuity. I hereby give permission for my child to be photographed for media and/or presentation materials to promote Elite Dance Center.

I understand the dance studio is a classroom. Please no disruptions during class. Students should come into class fully prepared with hair properly secured and dance shoes on. Please check the studio for the proper dress code. Siblings of students are not to be left unsupervised in the playroom or waiting areas. Please do not allow children to touch or play with the TV or trophy shelves. Kindly help us keep the playroom neat by returning all toys to the bins, dispose of any trash in proper receptacles and clean up any spills.

I understand the nature of this Activity and believe that my child is mentally qualified and in proper physical condition to undertake the Activity. I fully understand the risks incidental to participation in such Activity and that this Activity involves risk of serious bodily injury, which may be caused by my child's own actions or inactions, those of others participating in the event, the conditions in which the event takes place or the negligence of the Releasees. I acknowledge that there may be other risks either not known to me or not readily foreseeable at this time and I fully accept and assume all such risks and all responsibility for losses, costs and damages I incur as a result of my child's participation in the Activity. In consideration of my child (the "Minor") participating in dance lessons and related activities (the "Activity") at Elite Dance Center, Inc., I, the Minor's parent or legal guardian, hereby release, dis-charge and covenant not to sue Elite Dance Center, Inc., its owner, director, agents, officers, volunteers and employees, other participants, any sponsors, advertisers and/or owners of premises where the Activity takes place (collectively "Releasees") from all lia- bility, claims, demands, losses, damages, costs, expenses, lost wages and loss of services of any kind whatsoever for personal injury and/or property damage, known or unknown, which may result from my child's participation in, preparation for, or any other activ- ity associated with Elite Dance Center, Inc., whether arising before, during or after such activities, including negligent rescue oper- ations. I further agree that if, despite this release, I, the minor or anyone on the minor's behalf makes a claim against any of the above Releasees, I will INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expense, attorney fees, loss liability, damage or cost any Releasee may incur as the result of such claim. I understand that the art and nature of dance instruction may sometimes require a teacher to touch my child. I understand that this will happen in a gentle, caring and appropriate manner. I hereby grant permission to administer first aid help and/or call 911 in case of emergency while attending classes, rehearsals and performances or events on or off site.
