The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Dance Theatre of Coastal Virginia, LLC has put in place preventative measures to reduce the spread of COVID-19; however, Dance Theatre of Coastal Virginia, LLC cannot guarantee that you or your child(ren), or family members will not become infected with COVID-19. Further, attending Dance Theatre of Coastal Virginia, LLC could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Dance Theatre Coastal Virginia, LLC and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Dance Theatre of Coastal Virginia, LLC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Dance Theatre of Coastal Virginia, LLC employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Dance Theatre of Coastal Virginia, LLC or participation in programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Dance Theatre of Coastal Virginia, LLC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Dance Theatre of Coastal Virginia, LLC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Dance theatre of Coastal Virginia, LLC program.

I hereby agree to assume all risks attendant upon myself and/or my child while participating in any class at the Dance Theatre of Coastal Virginia, LLC. I hereby waive, release, and discharge any and all claims for damages for death, personal injury or property damage which I or my child may have, or which may hereafter accrue to me or my child, as a result of my child’s participation in any class at the Dance Theatre of Coastal Virginia, LLC. I agree to save and hold harmless from liability the Dance Theatre of Coastal Virginia, LLC. and any of their teachers or volunteers by reason of death, injury or damages to persons or property, which myself and/or child may suffer while participating in class.

I acknowledge and consent to allow the Dance Theatre of Coastal Virgnia,LLC to use: photos and/or videos of myself and/or child/ in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio’s website, or on social media sites (such as the studio’s facebook or instagram account). I understand that in some cases, my mine and/or my child's name may be used. I further understand no financial compensation will be for use of these photos/videos.

If I have any issues and will permit only for specific circumstances, I am acknowledging that I will communicate my preferences to Joy Naik.

I fully understand that at the Dance Theatre of Coastal Virginia the owner, and instructors, are not physicians or medical practitioners of any kind. With the above in mind, I hereby release and grant permission to the Dance Theatre of Coastal Virginia, LLC's staff to render temporary first aid to myself and/or child in the event of any injury or illness, and if deemed necessary by the Dance Theatre of Coastal Virginia, LLC's staff to call emergency services to seek medical help.
