I acknowledge that dancing may involve strenuous exertion on the part of my child. I certify that my child is physically fit enough to participate in dance activities. I acknowledge that with dancing, as with any physical activity, there is a risk of injury. By allowing my child to participate, I knowingly and voluntarily assume all risk of injury, regardless of severity, associated with participation of my child in activities of the Kallie Fox Dance Academy, LLC (KFDA).

I hereby release Kallie Fox Dance Academy, LLC, and all of its employees from any liability from bodily injury or harm while on the premises of Kallie Fox Dance Academy, LLC or out on location when representing Kallie Fox Dance Academy, LLC.

I hereby consent to Kallie Fox Dance Academy, LLC providing first aid medical treatment to my child as may be deemed advisable in the event of injury, accident, or illness, during my child's dance activities. I hereby certify I have adequate health insurance to cover any injury or damages that my child may suffer while participating, or alternatively, agree to bear all costs or expense associated with any such injury or damages myself.

I understand that while my child participates in dance at Kallie Fox Dance Academy, LLC that I and my child may be photographed. I agree that the photo, video, or film likeness of me or my child is the property of and may be used for any legitimate purpose by Kallie Fox Dance Academy, LLC. I grant permission to the Kallie Fox Dance Academy, LLC to use its photos and videos of my child/children for advertising and publicity purposes, such as print advertising, educational videos, television, social media platforms, recorded video, or film broadcast in connection with its promotional campaigns, without the payment of royalties or other compensation to me or my child. I acknowledge that photography and videography by parents/guardian or other family representative(s) of classes and/or events associated with Kallie Fox Dance Academy, LLC is prohibited unless approved otherwise by Kallie Fox Dance Academy, LLC.

A valid credit card is required to be kept on file in the KFDA Jackrabbit system for event payments. Published event registration fee includes the costs for credit card service transaction processing.

I hereby authorize Kallie Fox Dance Academy, LLC to charge my credit card on file for the event registration amount .

If a credit card is declined, your account will be charged a $40 Non-Sufficient Funds (NSF) Fee. Declined cards must be resolved within 5 business days.

All event fees are NON-REFUNDABLE. KFDA reserves the right to have a dancer sit out of the event for ANY outstanding account balances or improper behavior. All outstanding balances must be paid in full before the event. Accounts with a history of non-payment or non-compliance with studio policies may be barred from registration and may be asked to leave the studio. Please note the KFDA Online Store return/exchange policy.

I acknowledge that I am responsible for the behavior and actions of my child during the event. KFDA requires your child to respect our dance studio by behaving properly including but not limited to: listening and following the dance instructor's directions, respecting the instructor and other students, not eating in the studio, not running around, not using vulgar language, not causing damage to the facility or dance equipment, and keeping the studio clean. If a student cannot properly behave, the student will be pulled out of the event and his/her parent will be contacted to pick him/her up from the studio. Behavior issues may result in a parent being asked to remove their student from the studio until he/she is mature enough to return.
