The undersigned, on his or her behalf, and on the behalf of the minor(s) identified below, or on my own behalf if 18 years of age or older, acknowledges, appreciates, understands, and agrees to the following: I represent that I am the parent or legal guardian of the minor(s) named below or I have obtained permission from the parent/legal guardian of the minor(s) named below to execute this agreement on their behalf, or on my own behalf If 18 years of age or older. I have read the registration information and understand Princeton Ballet School's calendar and policies, as outlined.

I certify that this minor, or myself if 18 years of age or older, is in good health and capable of participating in all school activities and classes. I acknowledge and understand that there are known and unknown risks associated with participation in Princeton Ballet School activities. I agree that, on my own behalf and on behalf of the minor(s) named below, I fully accept and assume full responsibility for all of these known and unknown risks, and I assume full responsibility for personal injury to myself and/or the minor(s) listed below, including, but not limited to, injury related to asthmatic reactions, allergies, and muscular or orthopedic injuries. I understand and agree that, on behalf of myself, the minor(s) listed below, and our respective heirs, assigns, administrators, personal representatives, and next of kin, I fully waive, release and forever discharge Princeton Ballet School, the Princeton Ballet Society, American Repertory Ballet and their officers, trustees, members, agents, assigns, representatives, faculty members and employees, from liability for any and all injuries, illness and/or loss arising out of or related to participation in Princeton Ballet School.

In the event of a medical emergency, Princeton Ballet School has my permission to authorize first aid and/or lifesaving treatment to the participant listed on this form. In the event of an emergency, every effort will be made to contact me immediately. If I cannot be reached and emergency care is necessary, Princeton Ballet School will call an ambulance to transport the participant to the closest hospital. Princeton Ballet School will not be responsible for any medical charges incurred.

I hereby give permission for Princeton Ballet School to take photographs and video recordings for use in the organization's materials online, television, radio, and print, without any compensation to the participant.

I understand that Princeton Ballet School cannot be responsible for any lost or stolen items.

I understand that makeup classes only apply to the full academic year classes and do not apply to all programs including, but not limited to, the follow: Hand in Hand, Open Enrollment, Pre School Residency, Summer Intensive, Summer Classes. I understand that all class makeups must be done within the current academic year or registered session at the discretion of the School Director.

I understand the policies here as well as the policies on the American Repertory Ballet / Princeton Ballet School website regarding studio policies, attendance policies, evaluation procedures, information for parents, studio closings, etc.

The School Director reserves the right to change the class schedule or faculty as necessary, including cancelling any class that does not have sufficient registration.

A $30 fee will be assessed for each returned check.

All programs may be a hybrid of in-person instruction (pending CDC, state, and local health recommendations) and online classes via Zoom. This format would allow for small class sizes and personalized instruction while still providing all dancers access to our beautiful studios. I understand that refunds/credits will not be given if the way classes are offered changes to in-person, virtual, hybrid, etc during the school year or session.

I understand that all tuition and fees due will be AUTOMATICALLY charged to my credit card on file on or after the first day of the class. I understand that all payments are non-refundable. I understand that I am responsible for maintaining current credit card and contact information on my Jackrabbit account.

I understand the tuition rates on the American Repertory Ballet / Princeton Ballet School website and the tuition and fees due for the classes and sessions that me or my students are registered for.

There are no refunds on tuition. Adjustments may be made only under exceptional circumstances and upon receipt of written notification by the Director and are subject to the approval of the Board of Trustees.

I understand that a valid credit card is required to be kept on file with Princeton Ballet School at all times. I will not refute charges from Princeton Ballet School made to my credit card on file.

If my account has a past due balance, I understand that this minor, or myself if 18 years of age or older, may be restricted from American Repertory Ballet / Princeton Ballet School activities including, but not limited to, the following until my account no longer has a past due balance: participating in classes and/or rehearsals and/or performances, being cast in dances and/or performances, receiving performance costumes, registering for future activities, and receiving an evaluation. Furthermore, I understand that other restrictions may apply at the discretion of American Repertory Ballet / Princeton Ballet School and that I will not be reimbursed or allowed make up classes for any activities that this minor, or myself if 18 years of age or older, misses due to a past due balance on my account.

CHARGE AUTHORIZATION

I authorize American Repertory Ballet / Princeton Ballet School to AUTOMATICALLY charge my credit card on file for the full tuition and fees for the class(es) I registered for.

Furthermore, I understand that the Tuition Policy laid out above continues to apply. I understand that this applies to all current and future enrollments performed through the American Repertory Ballet / Princeton Ballet School Jackrabbit Parent Portal or with an American Repertory Ballet / Princeton Ballet School representative.
