I understand that my child(ren) must be ACTIVE on my HAC membership to participate in this event. Attendees must also be age 5 with full bathroom skills.

This is a drop-off event for HAC members only. I understand that I must pick up my child(ren) at the SOUTH entrance promptly at the end of the event. Parents who do not pick up their children on time will incur a $20 fee every 15 minutes.

Children who demonstrate behavior that is unsafe for themselves or for other children participating will be given a warning. If the behavior persists, I understand that I may be contacted to have my child(ren) removed from the event for the duration.

I understand that I must sign my child in when dropping off at the SOUTH entrance. I must sign out and present a photo ID at the time of pick-up.

I understand that no refunds will be given 48 hours of the event.

For valuable consideration received, I grant to Hockessin Athletic Club and Stage Stars Dance and Acro the absolute and irrevocable right and unrestricted permission concerning any photographs that any photographer on Hockessin Athletic Club's or Stage Stars Dance and Acro's behalf has taken or may take of me or my children or in which myself or they may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade, without restriction as to alteration; and to use my name in connection with any use if Hockessin Athletic Club or Stage Stars Dance and Acro so chooses. I release and discharge Hockessin Athletic Club and Stage Stars Dance and Acro from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Hockessin Athletic Club.

I hereby, for myself, my heirs, executors, assigns and administrators, waive, release, and forever discharge Hockessin Athletic Club and Stage Stars Dance and Acro, their respective agents, sponsors, representatives, successors and assigns from any and all claims, demands, actions or rights of action of whatever kind of nature either in law or in equity arising from, or by reason of any bodily injury, death or property damage resulting from my participation in exercise programs or activities of any kind in connection with the Hockessin Athletic Club and Stage Stars Dance and Acro, whether by negligence or not. This waiver appears to this visit and all future visits.
