Participants will specifically assume all risks of injury arising out of his or her presence on or about the premises, or his/her use or intended use of equipment or facilities, or his/her participation in the activities, including attendance of K-Pop Dance Camp, of Studio 50 Gym, Dance & Cheer Center, LLC. I am fully aware that any activity involving motion, height or athletic activity creates the possibility of injury, including but not limited to scrapes, cuts, bruises, sprains, breaks, muscle injuries, concussions or other serious injuries. I fully understand and accept these risks as well as that are unknown to me upon the signing of the Agreement that may result in medical assistance, medical expenses and medical emergencies. I hereby release Studio 50, including, its officers, owners and employees from any liability of any such injury. I further agree to hold harmless, indemnify and defend Studio 50, including its officers, owners, and employees from any loss, liability, damage or cost incurred by them at any event at Studio 50. This release includes but is not limited to any claims of negligence, dangerous conditions, latent defect, premises liability, code violations, negligent security, failure to warn, vicarious liability, negligent hiring, supervision, maintenance or improper/dangerous equipment; it is intended to be as broad as permissible under Illinois law. Parents or Guardians signing this Contract assuming the risk and releasing claims as herein stated are doing so on their own behalf and on behalf of the minor child or children they are signing for, including their respective Estates, Guardians, and Representatives.
