DISNEY PARADE Camp at TOLUCA LAKE for ages 3-6yrs (Half Day)! Mon, July 14 - Fri, July 18th. HALF DAY (9:00am-1:00pm).
WE WILL CHARGE ACCOUNTS 7 DAYS PRIOR TO THE START OF CAMP - IF YOU NEED TO CANCEL, PLEASE DO SO MORE THAN 7 DAYS IN ADVANCE.
*Late Pickup: We allow a 10 minute grace period after class is over. At 10 minutes, we charge $5, plus $1 for every minute after that, until your child is picked up. This allows us to pay our staff to wait with any leftover campers!
*No Symptoms / Sickness Policy (if your child has chronic/lingering symptoms, please email us a doctor’s note prior to your arrival that clearly states your child is NOT contagious or infected with Covid-19 and is cleared to attend camp); Otherwise, any children with symptoms will be asked to go/stay home.

I, the parent/guardian of my child(ren) listed above, acknowledge that In allowing my child to participate in Blue Starz Productions/Petite Performers activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Michelle Cardno DBA Blue Starz Productions/Petite Performers & it's employees of any and all liability which may arise as a result of my child's participation in activities at Blue Starz Dance & Theatre School, during class, drop-off or pick-up. I understand Instructors will often need to make hands-on contact to adjust students' alignment and to demonstrate proper motion. I will keep the faculty informed of any recent illness, injury, or other condition that may interfere with their ability to dance in class.

Tuition/Fees & Cancellation

I understand that camp tuition will be charged 7 DAYS PRIOR TO THE START OF CAMP. I understand that if I need to cancel my Camp reservation, I must do so MORE THAN 7 DAYS PRIOR to the start of camp by sending an email to info@bluestarzproductions.com.
Otherwise, cancellation requests made less than 7 days prior to the start of camp will not be refunded, unless we are able to fill and resell your spot.

(The Blue Starz team loves to share some Blue Starz love every now and then on social media or marketing platforms. If you do not wish for us to include your child, that is okay. Please email us so we may make a note on your account.)
If we do not hear otherwise, I, the undersigned, give permission for Blue Starz Productions / Petite Performers to use video footage and / or photographs of my child(ren) listed above. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to: printed materials (eg - brochures and newsletters), online and offline advertising and promotion, videos and digital images such for use on Social Media.
I acknowledge that I am giving unrestricted permission for my child's image to be used in print, video, and digital media. I agree that these images may be used by Blue Starz Dance & Theatre School/Petite Performers for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
