DADA On Pointe is an exciting 3-day Ballet intensive created by Debbie Allen where beginner to advanced ballet students ages 9yrs-18yrs are invited to study with ballet masters from the U.S. and abroad. (min. of 3yrs experience )

Thursday, April 2nd, Friday, April 3rd & Saturday, April 4th - 9am-4pm daily. $425 tuition, $125 non-refundable deposit due March 8th, $300 Balance due March 21st. Payments received after March 21st will incur a $50 late fee. Late Registrations after March 25th - Tuition $475.

Single Class $50 plus $25 non-refundable Registration Fee.

HOLD HARMLESS AGREEMENT

As a participant of the Debbie Allen Dance Academy “DADA on Pointe” I hereby assume all risk associated with participation in “DADA on Pointe.” I agree to indemnify, defend and hold harmless the Debbie Allen Dance Academy, their agents, employees, officers, directors, successors and assigns. I fully and forever release and discharge such parties from any and all claims, demands, actions, causes of actions, suits, controversies and liability of any kind and nature whatsoever (collectively referred to as Claims) relating to personal injury or property damage sustained, incurred, or caused by aforementioned program or any other events and occurrences while said person is engaged in the Program or any other activity related thereto.
The foregoing assumption of risk, aforementioned to indemnify and release shall further be inapplicable to and of no force and effect for any and all Claims asserted against any third party independent contractor performing services for compensation in connection with the Activities, except to the extent and amount that such third party independent contractor has a valid right, and claim against the Debbie Allen Dance Academy, their agents, employees, officers, directors, successors and assigns.

That I/We authorize DADA to photograph, film, videotape, record or otherwise capture in any media the above-named student’s dance training, instruction, and performances and to use such recordings for instruction, promotion, publicity and broadcast uses. All ownership and copyright, title and interest in these recordings shall belong to DADA. I/We further grant DADA the right to use the above-named student’s name and bio, portrait, picture, likeness and recordings thereof for the purposes of advertising, publicizing, and promoting DADA;

STUDENT AGREEMENT

THIS AGREEMENT is made and entered into by and between the Debbie Allen Dance Academy, as defined in this Agreement, and the parent or guardian (collectively, "Parent") of the Debbie Allen Dance Academy student listed below ("Student").

RECITALS

A. The Debbie Allen Dance Academy is operated by Debbie Allen Dance Inc., a 501(c)(3) California nonprofit corporation located at 1850 S. Manhattan Blvd., Los Angeles, CA, 90019 (the "Company.")
B. The Debbie Allen Dance Academy, the Company, and its directors, officers, employees, agents, licensees, independent contractors, volunteers, successors and/or assigns, are hereafter sometimes individually and collectively referred to as "DADA."
C. From time to time it will be necessary for DADA to arrange for the services of independent contractors to manage and/or operate DADA as a dance/artistic training and instruction institute.
D. The Student seeks to engage DADA to provide intensive professional training and performance experience in the art of ballet, pointe, character, men’s technique, variations, modern, Graham, jazz and contemporary repertoire, as well as ballet history, and other artistic training (collectively, "Artistic Technique"), upon the terms and conditions contained herein.
NOW, THEREFORE, in consideration of the recitals above and the promises and covenants contained herein and other valuable consideration, the receipt and sufficiency of which are hereby acknowledged, Parent and DADA agree as follows:

1. Term. The term of this Agreement shall begin on the effective date of this Agreement and shall continue until the Intensive ends or until terminated by DADA or the Parent in accordance with the terms and conditions of this Agreement or by law.
2. Assumption of Risk. Participation at DADA carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from (i) minor injuries (ii) major injuries to (iii) catastrophic injuries. Parent hereby acknowledges that Student will participate in rigorous training and will be physically touched from time to time during his/her dance instruction. This may include, but is not limited to, lifting, grabbing, thrusting, turning, pushing and pulling (collectively, "Physical Touching"). Parent is fully aware of the risks connected with participating in DADA Instruction, Productions and Activities, as defined in Section 3, and Parent voluntarily assumes full responsibility for these risks.
3. Waiver of Liability. Parent agrees to release, waive, discharge and covenant not to sue DADA from any and all liabilities, claims, losses, demands, or causes of action, except in the event of gross negligence of DADA, that may arise from or be related to any loss, damage, accident, illness, or injury, including death, which may be sustained by Student or Student's personal property while participating in (i) dance training and instruction of Artistic Techniques, ("Instruction"); (ii) on-site and off-site productions and lecture demonstrations, dance recitals, and performances (collectively, "Productions"); (iii) recreational, educational, swimming and other cultural and social events organized from time to time by DADA (collectively, "Activities"); special activities and/or one-time productions that may come up from time to time ("collectively, "Special Events"). Parent agrees to further release, waive, discharge and covenant not to sue DADA from any and all liabilities, claims, losses, demands, or causes of action, except in the event of gross negligence of DADA, which may arise from or be related to any loss, damage, accident, illness, or injury, including death, which may be sustained as a result of Physical Touching, by DADA or by students participating in DADA Instruction, Productions, Special Events and Activities.
