As the legal parent, guardian or adult student, I release and hold harmless Next Level Dance Center, its owners and operators from any and all liability, claims, demands and causes of action whatsoever, arising out of or related to any loss, damage or injury, including death, that may be sustained by the participant and'or the undersigned, while in or upon the premises or any premises under the control and supervision of Next Level Dance Center, its owners and operators or in route to or from any of said premises.

The undersigned gives permission to Next Level Dance Center, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions or conditions and/or declare the participant to be in good physical and mental health.

Photo Release

I hereby give Next Level Dance Center permission to publish photographs taken of my child or children and their names and likenesses, for use in the Next Level Dance Center's print, online and video-based marketing materials, as well as other Next Level Dance Center publications. I hereby release and hold harmless Next Level Dance Center from any reasonable expectation of privacy or confidentiality for the child and children associated with the images specified above. Further, I attest that I am the parent or legal guardian of this child or children and that I have full authority to consent and authorize Next Level Dance Center to use their likenesses and names. I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other Next Level Dance Center publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.
