In consideration of participating in the men's bootcamp and spiritual awakening experience, and for other good and valuable consideration, I hereby agree to release and discharge from liability arising from negligence of the releasees: The Inked Disciple, LLC. & Combat Ministries, Inc., DBA Freedom Martial Arts & Fitness, DBA Mentoring Warriors, and its owners, directors, officers, employees, agents, volunteers, participants, insurers, and all other persons or entities acting for them (hereinafter collectively referred to as “Releasees”), on behalf of myself and my children, parents, heirs, assigns, personal representative and estate, and also agree as follows:

1. I acknowledge that activities at the event (to include but not limited to extended periods of exercises, cold-plunge submersions in ice baths, sharing burdens and experiences with other men) may involve known and unanticipated risks which could result in physical or emotional injury, paralysis or permanent disability, cardiovascular risks, death and property damage. Risks include, but are not limited to, (a) bodily injury, partial and/or total disability, paralysis and death. (b) The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe.(c) These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named above.(d) There may be other risks not known to us or are not reasonably foreseeable at this time (e) medical conditions resulting from physical activity, and (f) damaged clothing or other property. I understand such risks simply cannot be eliminated, despite the use of safety equipment, without jeopardizing the essential qualities of the activity.

1a. I understand that new exercise routines and exposure to cold-water plunges contain health risks and will consult my physician prior to participating in the event.

2. I expressly accept and assume all of the risks inherent in this activity or that might have been caused by the negligence of the Releasees. My participation in this activity is purely voluntary and I elect to participate despite the risks. In addition, if at any time I believe the event conditions are unsafe or that I am unable to participate due to physical or medical conditions, then I will immediately discontinue participation.

3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Releasees from any and all claims, demands, or causes of action which are in any way connected with my participation in this activity, or my use of the equipment or facilities. This release does not apply to claims arising from intentional conduct. Should Releasees or anyone acting on their behalf be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

4. I represent that I have adequate insurance to cover any injury or damage I may suffer or cause while participating in this activity, or else I agree to bear the costs of such injury or damage myself. I further represent that I have no medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume – and bear the costs of – all risks that may be created, directly or indirectly, by any such condition.

5. In the event that I file a lawsuit, I agree to do so solely in the state where Releasees’ facility is located, and I further agree that the substantive law of that state shall apply.

6. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. By signing this document, I agree that if I am hurt or my property is damaged during my participation in this activity, that I have waived my right to maintain a lawsuit against the Releasees. I have had sufficient time to read this entire document and, should I choose to do so, consult with legal counsel prior to signing. Also, I understand that this activity might not be made available to me or that the cost to engage in this activity would be significantly greater if I were to choose not to sign this release, and I agree that the opportunity to participate at the stated cost in return for the execution of this release is a reasonable bargain. I have read and understood this document and agree to be bound by its terms.

I grant releasees the absolute and irrevocable right and unrestricted permission concerning any photograph or video that they have taken (or may take) of me, or in which I may be included with others, to use, reuse, publish and republish the photographs and videos in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, printed material, and for any purpose whatsoever, specifically including illustration, promotion editorial, advertising and trade, without restriction as to alteration; and to use my name in connection with any use if they so choose.

I release and discharge Combat Ministries, Inc. and The Inked Disciple Collective LLC. from any and all claims and demands that may arise out of, or in connection with, the use of the photographs and videos, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also insure to the benefits of the heirs, legal representatives, licensees, and assigns of the releasees.

I agree to opt-in in receiving communications from releasees through texting, emailing, and phone platforms.

This Non-Disclosure Agreement (“Agreement”) is entered into as of the date of official registration, by and between the releasees on behalf of The Forge - A Men’s Ministry Event, and the undersigned participant (“Participant”).

1. Purpose
The purpose of this Agreement is to foster an environment of authenticity, transparency, and trust during the Men’s Ministry event (“Event”), while ensuring that all personal, sensitive, or confidential information shared remains protected.

2. Definition of Confidential Information
For purposes of this Agreement, “Confidential Information” includes, but is not limited to:

Personal stories, testimonies, struggles, or experiences shared by any participant.
Prayer requests, counseling discussions, or spiritual guidance provided during the Event.
Any other information disclosed in a manner that a reasonable person would understand to be private.

3. Obligations of Participant
The Participant agrees to:
a. Maintain strict confidentiality regarding all Confidential Information shared during the Event.
b. Not disclose, reproduce, or communicate any Confidential Information to any third party without the express written consent of the person who shared it.
c. Use Confidential Information solely for personal spiritual growth and fellowship, not for gossip, judgment, or any harmful purpose.

4. Exceptions
Confidentiality obligations do not apply to information that:
a. The Participant is legally required to disclose under court order or applicable law.
b. Involves imminent risk of harm to self or others, in which case appropriate authorities may be contacted.

5. Term
This Agreement shall remain in effect indefinitely with respect to all Confidential Information disclosed during the Event.

6. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of Indiana, without regard to its conflict of law principles.

7. Entire Agreement
This Agreement constitutes the entire understanding between the parties regarding confidentiality for the Event and supersedes all prior discussions or agreements, whether oral or written.

8. Acknowledgment
By registering, the Participant affirms that they understand the importance of authenticity, transparency, and confidentiality, and agree to uphold these values in all interactions related to the Event.
