As the legal parent or guardian, I release and hold harmless Cheer Infinity Allstars, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Cheer Infinity Allstars, its owners and operators or in route to or from any of said premises.
Neither Cheer Infinity Allstars, its coaches and other staff members, will accept responsibility for injuries sustained by any student during the course of tumbling, cheerleading instruction, or open workouts, nor in the course of any exhibition, competition, or clinic in which he or she may participate or while traveling to or from the event.
With the above in mind, and being fully aware for the risks and possibility of injury involved, I consent to have my child or children participate in the program offered and further agree to waive all rights and claims for damages that I or m child may have against Cheer Infinity Allstars and or it representatives whether paid or volunteer.
I also affirm that I now have and will continue to provide proper hospitalization, health and accident insurance coverage, which I consider adequate for both my child's protection and my own protection.
I also understand that it is the parents' responsibility to warn the child according to what the parent feels is appropriate. Cheer Infinity Allstars will only warn child through "Safety Messages", our teaching style and progressions.
I also understand that during this camp my child will be leaving the facility of Cheer Infinity to go Jungle Rapids and I agree to hold harmless any damages that may occur.

I fully understand that Cheer Infinity Allstars staff members are not physicians or medical practitioners of any kind. With the above in mind,, I hereby release the Cheer Infinity Allstars staff to render temporary first aid to my child or children in the event of any injury or illness, and if deemed necessary by the Cheer Infinity Allstars staff to call our doctor and to seek medical help, including transportation by a Cheer Infinity Allstars staff member and or its representatives, whether paid or volunteer, to any health care facility or hospital, or calling of an ambulance for said child should the Cheer Infinity Allstars staff deem necessary.

I understand that I responsible for a Non-Refundable $75 Deposit for each child I register for Flip-N-Fun Summer Camp. I understand that under no circumstances can I request to be refunded the deposit made and I understand that under no circumstances will I be issued a refund for the deposit made. I understand that I must make the remainder of the payment prior to the week before the camp date.

In case of a rainy day, I give permission for my child(ren) to be transported either to or from Jungle Rapids by an employee of Cheer Infinity. Weather permitting, children will be walking to and from these venues from Cheer Infinity.

Cheer Infinity from time to time will use an athletes photograph on our website, social media and the TV's in the gym. If you do not give permission for us to use your athletes photograph, please stop at the front desk or send an email to make that note on your child's account.
