Welcome to the Fine Arts Academy! Regina's only Dance, Yoga, Fitness, Music, Art and Drama Preschool. Each day your child will be exposed to each of these Fine Arts elements while academically and socially preparing them for a successful start to Kindergarten. By filling out this form you are adding your child to a waitlist for the 2024-25 school year season. Fine Arts Academy Policies 2024/25 Season

I have received (below) a copy of Dance City Inc. Fine Arts Academy policies and have read and will take the responsibility to carefully follow the policies therein. Dance City Inc. Fine Arts Academy runs either at the physical studio / classroom locations or online. I understand that Dance City Inc. Fine Arts Academy does not give credit and/or refunds for class(es) missed for any reason, including due to holiday, vacation, illness, weather, closure due to pandemic, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Dance City Inc. Fine Arts Academy, and I voluntarily assume the risks associated with such participation in-studio and online.

DANCE CITY INC. STUDIO FINE ARTS ACADEMY POLICIES I, being the parent or legal guardian of a Dance City Inc Fine Arts Academy. student ("my child"), have received a copy of these Dance City Inc. Fine Arts Academy ("Dance City") policies and take full responsibility to carefully follow them and ensure that my child follows them. In registering my child, I confirm that I have read and understand the studio policies and I agree to be bound by them.

Dance City Inc. Fine Arts Academy Studio Policies: Registration Fee: A non-refundable deposit as well as a registration fee of $35.00 plus GST is due at time of registration.
Tuition: Fine Arts Academy

Tuition will be accepted in the form of 9 equal instalments. Class fees are payable the first of each month September to May.

General Policy / Payment Information:

Payments:
Automatic Monthly Credit Card Charges (Automatic/Recurring Payment Plan): This will be set up through the Studio Director upon registration.

I also authorize Dance City Inc. Fine Arts Academy to initiate electronic payments for the balances due on my Dance City Fine Arts Academy account each session on the first of each month.

Any time a payment is processed, I will receive an e-mail notification if I have not opted out of e-mail communications.

I understand that the payment amounts may vary as classes are added and/or dropped and as other charges/payments are applied to my account.

I acknowledge that this authorization will remain in effect until I notify the office by filling out a cancellation form provided by Dance City Fine Arts Academy that the authorization should be terminated (see cancellation policies).

If for whatever reason, payments cannot be processed with the above payment information and my account balance remains overdue, I understand that my child's enrolment in his/her classes will be cancelled.

The Registration fee and Deposit will be collected at the time of registration.

Payment Conditions:

Method of payment for class fees may be by Visa, MasterCard, or e-transfer; however, if paying by e-transfer, the customer agrees to have a valid Visa or Master Card on file.

Fees and charges for NSF cheques shall be applied to the credit card.
As of September 2024, 3% interest will be charged each month to all overdue accounts..

Rules and Regulations

Rules and Regulations:

- Dance City Fine Arts Academy is NOT responsible for any injuries that a student may sustain.
- Dance City Fine Arts Academy has the right to refuse or to suspend any individual who infringes any school rules at any point during the season.
- All newsletters and information will be sent via email. It is your responsibility to update your email address if there is a change by updating your account online.
- Dance City Fine Arts Academy is NOT responsible for any items lost or stolen.
- If a student will be missing from a class we ask that the parent or guardian inform their teacher or the studio through our parent portal.
- Parents are asked to respect drop off and pick up time to avoid classroom disturbance. Drop off between 9:00 - 9:10 (class begins at 9:15), pick up - 11:45.
- Waiting room will be open 15 minutes prior to class start time. Parents are asked to stay with their child during this time in our main floor lobby until your teachers child meets you to take your child to their classroom.
Holiday and statutory holiday closures are listed on our website events calendar and will be emailed out in our "year at a glance" after enrollment is complete.
Chewing gum or eating food is not permitted in class with the exception of snack time.
Dance City Inc. Fine Arts Academy reserves the right to combine or cancel any class.
Dance City Inc. Fine Arts Academy is a Nut/Peanut Free Facility.
I am aware that the teachers reserve the right to move my child to a different level if the one they are registered in is not the correct level
Disrespectful behavior towards myself, the Owner, Nadine Selinger, any of my staff members or students will not be tolerated. Students or parents behaving in this manner may be removed from the school immediately with no refund to fees.

I/We agree to all terms and conditions and hereby release and discharge Dance City Fine Arts Academy, its directors, employees, contractors, and teachers from any and all actions, causes of actions for damages, losses or injuries of any nature or kind which may be sustained by the student while attending and/or participating in any and all classes or programs conducted by Dance City. Dance City shall not be held responsible for any loss or theft of any personal possessions whatsoever. I acknowledge that I have read and understood the above policies and agree to them.

Cancellation

WITHDRAWAL POLICY FINE ARTS ACADEMY

- A parent/guardian must complete a cancellation form provided by Dance City Fine Arts Academy one month in advance of your child's withdrawal from a class or classes. You will find this form on our weekly update emails.
- After this notification in writing you are responsible for one additional month's payment (i.e. if you submit cancellation form to Dance City Fine Arts Academy on November 1st, you will pay for the November classes and your child will withdraw at the end of November) and the automatic monthly credit card charges will be canceled for subsequent months.
- The 7th of the month is the cut-off date to notify of withdrawal. Any notification after the 7th of a month will hold you responsible for that month's payment and the following month's payment ( i.e. if you submit cancellation form to Dance City Fine Arts Academy on November 8th you are still responsible for your November payment as well as your December payment even if it is your intention to withdraw your child(ren) at the end of November.)
- Cancellations are not accepted for any classes after March 1st, NO REFUNDS will be given for the remaining months.
- Registration fees and deposits are non refundable.
- Refunds are not provided for any missed classes.

GENERAL INFORMATION

- At any time Dance City Fine Arts Academy has the right to move all classes to an online format through zoom in the event that it is no longer safe to conduct in-person instruction.
- There will be no discounts or refunds given and all above cancellation policies will be in effect.
- Absence: Missed classes are non-refundable

Dance City Inc. Waiver Agreement

I, being the parent or legal guardian of a Dance City Fine Arts Academy student (referred to in this Waiver as "my child"), hereby acknowledge that in consideration of Dance City Inc. Fine Arts Academ ("Dance City") accepting my child as student, and for other good and valuable consideration, I for and on behalf of my child acknowledge and agree as follows:

1) I acknowledge and understand that there are risks that are inherent in my child's participation in dance, acrobatic dance and gymnastic activities in which he/she is registered, including classes and rehearsals (collectively, the "Activities"), including without limitation, the possibility that my child may sustain severe bodily injury, including permanent disability and death, and economic loss, property damages, and losses of any other kind and nature, which might result not only from my child's own actions, inactions or negligence, but the actions, inactions or negligence of others or use of any equipment. I further acknowledge that there may be other risks not known or not reasonably foreseeable as at the date of signing this agreement. All of the foregoing risks mentioned in this paragraph are referred to in this agreement as the "Risks".

2) I voluntarily assume all of the Risks and accept personal responsibility for any and all loss, damage, expense (medical or otherwise) or injury, including permanent disability or death, that my child may suffer as a result of my child's participation in the Activities. I understand that the Risks simply cannot be eliminated without jeopardizing the essential qualities of the Activities.

3) I release, waive, discharge and covenant not to sue Dance City, its officers, directors, shareholders, employees, contractors, agents and representatives (collectively, the "Releasees") from any and all liability to my child and my child's next of kin for any and all claims, demands, losses or damages on account of injury, including permanent disability, death, or damage to property (collectively, the "Claims") caused or alleged to be caused in whole or in party by the Releasees negligence, breach of any duty of care, including any statutory duty, breach of contract, or any other act of the Releasees as a result of or in connection with the Activities.

4) I agree to hold harmless the Releasees from any and all claims of any third party resulting from my child's participation in the Activities.

5) I acknowledge that I am entering into this agreement voluntarily and that in entering into this agreement, I am not relying on any oral or written representations made by the Releasees with respect to the safety of participating in the Activities, other than what is set out in this agreement.

6) I certify that my child has no medical or physical conditions which could interfere with my child's safety in the Activities and that my child is in proper physical condition to participate in the Activities.

7) If my child sustains an injury or illness and requires medical attention at a time that I and, if applicable, my child's other parent or legal guardian cannot be contacted, I hereby authorize Dance City to seek medical services for my child in that case. I further agree to accept financial responsibility for any and all costs associated with this authorization and to indemnify Dance City for any such costs paid by Dance City.

8) I further grant Dance City permission to use my child's likeness in photographs and videos understanding that Dance City may utilize such images for its own purposes, including the publication of such photographs and videos in information circulars, advertising materials, its website, and in its staff training materials, with all such photographs and videos becoming property of Dance City. It is my further understanding that Dance City will not, without my prior written consent, publish my child's name or any other personal information in Dance City's use of such photographs and videos.
