Starlet Summer Dance Camps are designed for dancers ages 3-7yrs old. Get ready to take off at our Beach Party Blast with Barbie, Ken and friends! This wondrous camp delights in summer FUN with dance, movement and imaginative play. We'll surf into sporty style, hang ten, and have a Go-Go good time. I understand that my Card on File will be charged for fees due within 24hrs of enrollment. I understand that there are no Camp refunds and that 48 hours notice for cancellation is required to receive credit (minus $15.00 cancellation fee). There are no transfers of Camps.

While attending classes/camps at RAD Dance Studio (In Studio or Online/Virtual) , there is always the possibility of injury/illness. RAD will provide responsible care for my child, but, if some injury/illness should occur, I, the undersigned agree not to hold RAD, the owners, any of the chaperones, or any of its staff responsible. I further agree to indemnify and hold harmless RAD, and its agents and employees from claim, losses, injuries, and damages of any nature whatsoever incurred. I further authorize the agents of RAD to obtain any and all necessary emergency health care, in my stead, for my child. By checking this box you are confirming that you have provided RAD with the medical insurance carrier and policy number for you child as well as any allergies, medications, or previous injuries that may be pertinent to you child's participation in dance.

I am aware that RAD Dance Studio has implemented stringent cleaning and disinfectant policies to do its best to sterilize its facility. I will hold N & R Productions/RAD Dance Studio harmless if sickness due to COVID-19 or any other viral or bacterial illness is traced back to RAD Dance Studio classes/rehearsals/auditions/any event.
