As the parent or guardian of the above referenced child(ren), I hereby give permission for our child(ren) to participate in Movie Night at PDA USA (Pacific Diving Academy USA, Inc.) and SAGA (Straight A’s Gymnastics Academy, LLC). I recognize that this is a drop off event that involves jumping in foam pits, climbing ropes, using gymnastics equipment, and participating in other gymnastics and/or high-energy activities and that there are inherent risks involved.

On behalf of my child and on our own behalf, I agree to waive all claims against Pacific Diving Academy USA, Inc. and Straight A’s Gymnastics Academy, LLC and it’s owners, staff and instructors for any liability, loss, cost, damage, medical expense, long-term care or emotional distress arising out of any personal injury, including total disability, paralysis and death, which may occur to any of the children while on the premises of or under the instruction, supervision, or control of Pacific Diving Academy USA, Inc. and Straight A’s Gymnastics Academy, LLC. I hereby testify to my child’s sound health of mind and body and authorize to seek medical treatment at the nearest medical facility in case of emergency.

I hereby grant consent and authorize the use of photographs, slides, video recordings of my child participating in Pacific Diving Academy and Straight A’s Gymnastics Academy activities for commercial and purposes in any medium of advertising, communication, publication or publicity that will promote Pacific Diving Academy and Straight A’s Gymnastics Academy, and/or recognition of participants.

I have read and understand all the above and agree to the above terms, including the Risk and Waiver of Liability.

All children are expected to be picked up by a parent or authorized guardian by the end of class/practice or a PDA/SAGA event (including but not limited to PNO, open gym, camp, birthday party, etc.). All families will be granted a 15-minute grace period after the end of their child's designated activity.

If a parent/guardian arrives even one minute after the end of the grace period, families will be charged a designated late fee of $25. After 30 minutes, a $1 per minute late fee will be charged. There will be no exceptions or warnings.

If a parent/guardian is late for whatever reason (flat tire, heavy traffic, weather conditions, etc.) a late charge will be issued. A "no exceptions" policy makes it easier to apply the late policy to everyone consistently and fairly.

Fees for late pick-up are payable immediately; if not paid, the child will not be readmitted to the gym.

There are no refunds of any kind. If you sign up for Movie Night, and do not show up for the event, no refunds, credits, or exchanges will be given.
