I hereby indemnify, hold harmless, release, waive, discharge and covenant not to sue Short Dance Studios, Inc. including its directors, officers, agents, and employees, all for the purposes herein referred to as "Releasees," FROM ALL LIABILITY, TO THE UNDERSIGNED AND THE STUDENT, FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIMS OR DEMANDS THEREFORE ON ACCOUNT OF INJURY TO THE STUDENT OR PROPERTY OR WHILE IN THE ACT OF BEING TRANSPORTED TO AND FROM SAID ACTIVITY OR ACTIVITIES RELATED TO SHORT DANCE STUDIOS INC. OR COST they may incur ARISING OUT OF OR RELATED TO THE EVENT(S), WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. I understand that by agreeing to this clause I am releasing claims and giving up substantial rights, including my right to sue.

I understand that activities at the Short Dance Studios Inc., include certain inherent risks that cannot be eliminated regardless of the care taken to avoid such injuries. Some of the risks include, but are not limited to: 1) minor injuries such as bruises, sprains or strains, to 2) major injuries such as broken bones, dislocations and torn muscles. I HEREBY Assert that my participation is voluntary and that I knowingly ASSUME FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY OR PROPERTY DAMAGE arising out of or related to the EVENT(S) whether caused by the NEGLIGENCE OF RELEASEES or otherwise.

*MINI CAMP TUITION - Mini Camp tuition is due at the time of registration. The studio will process your payment via the billing info. you provided. The studio must be notified of cancellation 10 days prior to the camp in order to get a refund. A full refund will be given if there is a cancellation on the studios part.
COLLECTION PROCESS - You will receive notices of unpaid fees before any collection action is taken. If there is no response to our notices, your account will be sent to collections after three attempts to notify you. In the event that we do not have updated contact information, Short Dance Studios Inc. reserves the right to send your account directly to collections without warning. If there is no payment(s) received or any payment plan agreed upon, your account will be forwarded to our collection agency. You will be liable for the unpaid fees on your account, PLUS ANY COLLECTION OR ATTORNEY FEES ASSESSED TO Short Dance Studios, Inc. or any of its programs.

I HAVE READ THE ABOVE INFORMATION AND ADDITIONALLY, I (WE) UNDERSTAND THAT PHOTOGRAPHS MAY BE TAKEN OF THE STUDENTS TO USE IN BROCHURES, WEB SITES, POSTERS, ADVERTISEMENTS AND OTHER PROMOTIONAL MATERIALS THE STUDIO CREATES. I THEREFORE GRANT PERMISSION FOR SAID PICTURES TO BE USED BY THE STUDIO FOR SUCH PURPOSES. I GRANT THE STUDIO PERMISSION TO COPY-WRITE SUCH PHOTOGRAPHS IN ITS NAME.
