I am aware that my actions as a parent or legal guardian will impact the safety of others in the Sunrise Gymnastics facility. Sunrise is counting on me and all the other parents and kids to help keep us all safe by meeting the physical-distancing and personal hygiene standards recommended by CDC and the State of Vermont.

As a consequence of the restrictions placed on Sunrise by the Vermont Department of Health and our commitment to safety, parents must agree to follow the following policies:

1. Students must use the restroom and wash their hands thoroughly before leaving for the gym. Students should always wash both hands and feet after returning home. Hand sanitizer will be provided throughout the gym and waiting areas.

2. Only one parent may enter the gym with their child. Parents are welcome to drop off and pick up on the side of the gym using our new entrance.

3 . Mask ARE mandatory for all individuals, regardless of vaccination status * UPDATED 8/19/2021*

4. The entry area and seating/ waiting area will have clearly marked physical-distancing position indicators. If this space starts to fill up, kindly move or wait outside and we will walk your child out to meet you. (Just leave a note on the front desk with your child's name)

5. Please DO NOT send your sick child or yourself into the gym if you are not feeling well. We offer make-ups.

6. Please note that every child who enters the gym will be scanned for their temperature and if we find anyone with a slightly higher temperature, we will contact you to bring your child home and they will not be allowed to enter the gym until they are symptom-free.

I will do my part to help Sunrise Gymnastics stay safe by being respectful of the gym's social-distancing policies, insisting that my child wash their hands before entering the gym and when arriving at home, and by supporting the amazing teachers, coaches, and customer service staff as they work hard to provide a safe place to develop Happy, Healthy, and Confident Kids.

Waiver

In consideration of the services of Sunrise Gymnastics, Inc., their agents, owners, officers, volunteers, participants, employees, and the other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as "Sunrise Gymnastics”), I hereby agree to release and discharge Sunrise Gymnastics, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows:

I acknowledge that the activities involved in the use of any of Sunrise Gymnastics’ services or facilities entail significant risks, both known and unknown, which could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties.
I expressly agree and promise to accept and assume all of the risks existing in these activities, both known and unknown, whether caused or alleged to be caused by the negligent acts or omissions of Sunrise Gymnastics. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks.
I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Sunrise Gymnastics from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of Sunrise Gymnastics equipment or facilities, including any such claims which allege negligent acts or omissions of Sunrise Gymnastics.
Should Sunrise Gymnastics or anyone acting on their behalf, be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I have no medical or physical conditions which could interfere with my safety in this activity, or else I am willing to assume - and bear the costs of - all risks that may be created, directly or indirectly, by any such condition.
